Facility was clean and organized.-Handsink clean and stocked.-Restrooms clean and stocked.-Alcohol advisory posted.-Ice machine clean and scoop stored properly.-Customer ice stored properly in separate bin. Observed good scoop storage. -Ice plate cleaned once per week; but area is not used for customer ice.-Speed guns clean.-3 compartment sink set up properly. Sanitizer at 300 ppm Quat. -Walk in cooler at 41F. Lights properly shielded. Dairy dates good. Observed mayo; mustard; catsup; relish; etc on a separate shelf in cooler. Per manager these items are for employee use only.-Mop area clean and organized.-Observed proper chemical storage.

    Noted mold formation inside top of ice machine. Clean and sanitize ice machine on frequent scheduled basis to keep free of mold. Ice machine to be cleaned and sanitized by 7/9/12 per employee.

    Thermometer inside walk-in cooler at 65 F. Per bartender; compressor turned off a few minutes earlier due to frost build-up on refrigerant line. Make necessary repairs on unit to maintain ambient temperature of 40 F or below at all times. Do not stock any milk products in walk-in cooler until it's properly operating.

    Noted top lid of ice machine to be missing. Provide lid to keep ice machine interior enclosed.
