Display freezers ok - all product frozen-downstairs walk in cooler at 41F-dated checked ok; discussed first in; first out procedures with no issues noted-thermometers available-restrooms stocked and clean-lights shielded-facility does not sell dented cans-dumpster ok-general cleanliness good-ice properly labeled-regular pest control by Clark; no pest issues noted at time of inspection-discussed employee health - ensure anyone with gastrointestinal symptoms (vomiting/ diarrhea) is excluded for 48 hours after symptoms stop.

Repair wall to be smooth; durable; and easily cleanable. Observed several holes in drywall throughout basement warehouse. Patch holes in walls to be smooth and easily cleanable. Recommend using FRP Fiberglass reinforced plastic or sheet metal to cover warehouse walls from damage caused from pallet jacks and operations of warehouse.

    Walk in units in basement warehouse checked OK and within regulations. Food properly stored. No raw foods below ready to eat foods. -10 walk in freezer. Do check to make sure lights are shatterproof bulbs overhead in walk in units. Bathrooms observed clean and stocked.Grocery store does not sell any damaged food products to customers. Food is rotated on shelves first in first out and shelves are dusted on a routine basis. Employee breakrooms observed clean and organized.Facility has active pest control operator that services premise routinely. Chemicals properly stored away from food. Garbage area and compacter observed clean and organized.

    All violations corrected at time of inspection. Thank you.All food throughout facility stored off ground. Chlorine/bleach test strips available for every food department. Please review with each department manager the proper makeup of bleach sanitizer and how to test sanitizer solution to be at levels between 50-100 ppm.Separate container for storage of scoop for bulk foods. This facility is operating with active managerial control and premise is kept clean and organized-great job everyone! Keep it up.

    Observed boxes of food stored directly on ground in back warehouse and in walk in freezer. Food product stored on floor. Store all food product at least 6 inches off floor to prevent potential cross contamination.Please provide a container for proper scoop storage at roasted peanuts display so that the handle to the scoop does not rest in the peanuts.

    Observed no chemical test kits available. Facility must provide chlorine test kit to the following departments:1) Produce2) Meat3) Bakery4) KitchenReviewed with operator proper makeup of chlorine sanitizer. Use one cap of bleach to one gallon of water and test sanitizer solution to be at levels between 50-100 ppm. Do change sanitizer water at buckets frequently.

    Nicacio Lopez is CFPM for produce department M10080; exp. date: 01/10/16.Cold holding units checked OK. Grocery store uses first in first out method of rotation. Damaged food products are not sold to public. Dairy dates observed current. No raw foods stored over ready to eat foods.No chemicals stored next to food. Names of manufacturers of ice are labeled on ice bags. Bathrooms observed stocked and clean. Discussed with operator to adjust automatic water faucets to free flow water for at least 15 seconds once activated. Employees should be monitored for a proper 20 second handwash and at appropriate times. Discussed with grocery store manager using TIME as a public health control. Produce departments cuts melons and other fruits to be displayed on ice. Please monitor temperature of this fruit salad especially cut melons of any kind to be cold holding at 40 F or lower. The fruit salad or any other potentially hazardous food (food that can breed bacteria between the temperatures of 40 F to 140 F) cannot be stored at this temperature longer than 4 hours. A log or documentation of the time the food enters the temperature danger zone ( 40 F - 140 F) must be time marked for the time in and the time it is discarded four hours later..

Major infestation is gone; and routine monitoring and treatment with Clark Pest Control is occurring. Discussed several issues on site with Manager and PCO. Issues included the need to fill out the sighting report and for the PCO to accurately note down the activity level in his reports. This is necessary to ensure that problem remains abated. Also discussed with manager that any deficiencies in structure or operation that are noted on any Pest Control Report must be addressed immediately; as if it had come from the Health Department.Facility name has also changed to Marketon. All ownership is the same. Permits will be changed to reflect this.

    Facility has ongoing pest issues. Weekly service is being conducted as well as larger fogging. Discussed with operator that if problem is not resolved rapidly; then further enforcement measures will be taken. Discussed changing PCO if results are not favorable.

    Eggs (two pallets) stored at room temperature in warehouse downstairs. Several boxes of flan and chorizo (prepackaged) also stored in receiving area. These items must be placed under refrigeration as soon as possible after receiving. *Items moved*

    Some mice droppings found in corner where many unused items are stacked up. Facility does have pest control service that comes twice weekly; droppings may be old. Clean up droppings and maintain area so that harborage is not created.There is a bit of a fly issue in the facility. Both the meat market and the deli area bug zappers were non-operational and flies were observed. In addition; numerous gaps in door weather stripping and on roll up doors were observed.**Bug zappers and droppings must be cleaned up within 48 hours. Maintenance issues must be resolved within 1 week. **

    Floors in warehouse area below store need cleaning. One area (hole) has standing water that was clearly old. That area should be sealed. In addition; area under stairs needs a lot of cleaning. Various spills throughout.

    Many items that are not being used are not properly stored; leading to crowded and non-sanitary areas in warehouse area. All items not being used should be stored properly to allow for cleaning around and under them or stored off site.

    Overall; facility is clean; especially on store floor. Warehouse area underneath needs a little work.Product stored off floorRestrooms stockedChemical storage is separateAll refrigeration @ 40F or below**Temperature controls need to tighten throughout facility. Though most items were ok; most of the employees were not aware of/concerned with mataining items such as those listed above at safe temperatures. **Cleaning of warehouse needs to be a priority; as well as sealing entrances to prevent fly and/or vermin access. Suggested having a rotational schedule for pallets of product - such that one area stays clear and is cleaned each day. It is very difficult with the amount of product that flows through the facility.**Facility has had management change approximately 6 months ago. Only two certified food managers remain. Noted as a violation on the support kitchen permit. **60 days to obtain certified food manager for this permit. List of instructors provided.**
