Hands must be washed before putting on gloves.Noted ice machine interior clean.Pop corn machine interior is dirty; not in use. Clean and sanitize before using and remove grease build-up inside machine.

NOTES:1) Sheet vinyl located in some areas of the back bar floor. Appears to be clean and well maintained. The vinyl is currently not tacked down. It is recommended that the vinyl be secured to the floor for ease of cleaning. In the even the vinyl is not properly maintained; the Health District may require it to be installed correctly or removed.2) The Change in Ownership which occured in October of 2008 was approved with the beer tap drain running to a french drain located outside in the back of the facility. Advised operator that this drain may be required to be routed to the sanitary sewer system via a floor sink during future change in ownerships or if the outside drainage area becomes a nusance or health hazard.3) Recommend the owner/operator install base coving throughout the inside bar area or seal/caulk the bar to the concrete floor to prevent water and debris from accumulating under the bar and to protect the wood bar from damage.

The following items need to be fixed/corrected:1) Provide a container to store ice or beverages in cold plate area.2) Provide weather stripping at door next to unisex bathroom.3) Provide covered receptacles in women's bathroom and unisex bathroom. 4) Caulk all handsinks to wall in restrooms.5) Patch and seal all holes in walk-in reefer. Paint all non-porous surfaces to provide a smooth and easily cleanable surface. 6) Install nonporous base coving in walk-in reefer.7) Reseal or resurface missing tile areas in walk-in reefer.8) Remove all uneccessary articles from trash enclosure area. 9) Remove all unused equipment from bar.10) Paint any nonporous shelving that will store food or beverage items. 11) Replace broken switch plate in dry storage area. 12) Prior to opening provide soap and single service paper towels at all handsinks.Discussed with operator to contact Health dept. when reach-in cooler in bar area will be utilized. Discussed with operator that pre-packaged foods such as snacks can be served along with popcorn in non-communal bowls. Lighting adequate under 3 compartment sink. OK to issue permit.
