    -Soap dispenser at the hand sink in the womens restroom was broken. Soap was able to be dispensed; but you had to push and hold the top of the dispenser for it to work properly. Ensure soap dispenser is working properly within 7 days; 07/07/15; to allow proper hand washing.

Ensure employees sick with gastrointestinal symptoms ie. vomiting; diarrhea; jaundice; stay out of work at least 48 hours after symptoms have stopped.-Ice is from Arctic Glacier.-Thermometers provided.-All freezers observed at <0F-All coolers at 40F or below.-Observed good food storage.-Observed good chemical storage.-Mop area clean and organized.-Outside garbage stored correctly.-PCO is Steritech on a monthly basis.-No signs of pests or vermin.The facility also has the following CFPM's in addition to the CFPM's for each permit:Michael Herman.
