Consulation complete. Facility still under construction. Facility must complete the following before license is issued:
-Food prep sink and ice machine must have 2 inch air gap
-3 compartment sink must be pulled 4 inches away from wall
-install wire shelving above 3 comp sink to allow dishes to air dry.
-hand sinks must have proper signage and be supplied with soap and paper towels.
-hand sinks and food prep sink must be sealed to wall with 100% silicon cualk.
-all floors, walls, ceilings must be smooth and easily cleanable
-baseboards must be installed in entire kitchen area
-seal area behind food prep sink
-ensure all shevles are wire/smooth and easily cleanable
-ensure all surfaces are cleaned and sanitized.
-attend level 1 food safety class

Consultation for change of ownership. Observed walk in cooler holding at 58 F. Ensure cooler is repaired to hold 41 F or below. Esure the following is completed before next inspection:
-cooler is operating
-lights are shielded
-all hand sinks are supplied with soap and paper towels.
-mops are hung
-all food is 6 inches off the floor
-hand wash signs are posted at hand sink and employee restroom

Inspected and approved to operate as level 4. Collected check # 2269 for $327.00. Ensure the following items are completed before opening:
All hand sinks have soap and paper towels
Employee restroom has hand wash sign posted
Base molding installed in back room and front counter.
Ensure employee attends level 1 or servsafe.
