- COMPLAINT INVESTIGATION FOR ANONYMOUS COMPLAINT "MICE IN OPERATION."
- ARRIVED TO FACILITY AND DISCUSSED WITH GM AND CHEF(S).
- AT TIME OF INSPECTION, OBSERVED NO LIVE OR DEAD RODENTS IN THE FOOD OPERATION. CHEF(S) ALSO INDICATED NO RODENTS / PESTS HAVE BEEN SEEN IN THE CUSTOMER DINING AREA OR KITCHEN AREA OF THE FACILITY.
- FACILITY MAINTAINS ECOLAB AS FACILITY PEST CONTROL COMPANY. NOTED SEVERAL RODENT TRAPS LOCATED IN THE DRY STORAGE AND KITCHEN AREAS OF THE FACILITY. ALL TRAPS WERE VOID OF LIVE / DEAD MICE.
- OBSERVED OUTSIDE WASTE RECEPTACLE LITTER FREE AND CONTAINED.
- OBSERVED DOORS AND WINDOWS CLOSED AND PROTECTED FROM THE ENTRY OF PESTS / RODENTS.
- OBSERVED ALL FOOD PRODUCTS COVERED AND STORED 6 INCHES OFF THE FLOOR.
- MANAGER WERE UNAWARE OF ANY COMPLAINTS REGARDING MICE MADE TO THE FACILITY. ADDITIONALLY, NO KNOWN RECENT DISGRUNTLED EMPLOYEE WHO MAY HAVE MADE THE ABOVE ANONYMOUS COMPLAINT.
- OBSERVED SMALL AMOUNT OF FOOD / FLOUR RESIDUE UNDERNEATH BULK STORAGE CONTAINERS. FOOD RESIDUES SHALL

Observed employees in clean uniforms. All employees appeared well.

Observed all hand sinks appropriately stocked.
IV. Person in Charge Demonstration of Knowledge (+)
Person in charge is Serv Safe certified. All managers are Serv Safe certified. Managers provide 1 on 1 training to employees in food safety . Based on discussions manager is very knowledgeable in food safety.

Food comes mainly from GFS and Michael's Meats.

Food is thawed under refrigeration.

Foods were properly stored to prevent contamination.
