    Comments: EMPLOYEE'S GLOVES WERE DAMAGED AND HIS FINGERS WERE PROTRUDING THROUGH THE GLOVES. DISCARD -CORRECTED- A MONETARY FINE WILL BE ISSUED IF THIS HAPPENS AGAIN.

    Comments: EMPLOYEE WAS ASKED TO TAKE OFF GLOVES WHILE HE WAS LOOKING FOR A HAT. HE THEN PROCEEDED TO PUT ON A NEW SET OF GLOVES WITHOUT FIRST WASHING HIS HANDS - CORRECTED-HANDS MUST BE WASHED IN BETWEEN TASKS AND PRIOR TO PUTTING ON A NEW SET OF GLOVES.
