  • Basic - Ceiling soiled with accumulated dust. Observed on ceiling and wall above 3 compartment sink.
  • Basic - Ceiling tile in disrepair. Observed ceiling tile hanging by blue painters tape in kitchen area over 3 compartment sink drainboard.
  • Basic - Soiled reach-in cooler gaskets. Observed on doors of pizza make station.
  • High Priority - License is expired and is more than 60 days after expiration date. Observed license expired June 1, 2013. Expired license is displayed on wall near front counter. Inspector phoned D6 office and spoke with Kenny Hensdill who verified license is delinquent. **Admin Complaint**
  • High Priority - Dented/rusted cans present. Observed can of pizza sauce heavily dented in the middle near the bottom of the can with dent protruding into food product. Also observed two small dents on the top lip of the can parallel from each other. See stop sale.

  • Basic - Pizza pans/screens not cleaned at least every four hours.
  • Intermediate - No proof of required state approved employee training provided for employee hired more than 60 days ago. Observed employee working in kitchen with food who said his hire date was in December or January. To order approved program food safety material, call DBPR's contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. This violation must be corrected by August 12, 2013. **Warning** Callback 8/13/13: Observed Manager Paul on vacation. Employee Josh was on staff at time of inspection. He said he has only worked at location about 1.5 months with all other employees for establishment hired at that time at the latest as well. Inspector viewed 3 time cards for employees; Josh, Eddie and Michael. Josh called Manager Paul and was able to locate old SafeStaff certificates for 2 employees, Andrew and Brian, who are no longer employed at establishment. Since 3 employees at establishment were hired less than 60 days ago per employee Josh, manager has 60 days from the date new employees were hired to complete food safety training. This violation will be checked at next unannounced routine inspection for any new employees listed above (Josh, Michael, Eddie) and any other new employees.

Observed ground beef and ground sausage at 48°, wings at 48°, chicken tenders at 47° and cheese at 47°.
