Observation: The following areas have become soiled and must be cleaned: mixed fruit shelf, grape pop shelf, yogurt door, and the floors of the walk in coolers/freezers have food debris.
- Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
Observation: Observed raw beef and turkey stored above pork in the meat cooler. Must store all meat in order of internal cooking temp in coolers. Ready to eat food at the top, pork, eggs, seafood, then beef, and then chicken and poultry.
- Package integrity
Observation: Opened boxes of cereal and dented cans were observed in the retail area. May not sell any container that is dented, opened, or in poor condition.
- PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
Observation: Packaged and heads of lettuce were observed stored at room temp in the retail area. Lettuce was moved to the cooler. Cut leafy greens must be kept 41*F or below since the start of 2014.
- Heating, ventilating, and air conditioning systems design, installation, and cleaning
Observation: Walk in cooler (dairy) fan covers, and the walk in freezer vent covers (door side) have become dusty and must be cleaned.
- Light bulbs, protective shielding
Observation: Light bulbs in the entire facility do not have a cover and do not appear to be shatter resistant. Must provide either one for protection of contamination.
- Equipment location, installation, repair, and adjustment
Observation: Several door gaskets in the retail area are torn. Must replace door gaskets to ensure adequate temp is maintained.

Observed several dented cans in retail area. Must not sell any can that has been dented on the top, bottom, or side seals. Also any can that has sharp points due to denting or cannot sit level due to denting. Corrected at time of inspection

Observed mop heads bunched up in mop bucket. Must allow mop head to air dry when not in immediate use to prevent buildup of bacteria.

Could not locate thermometer in meat walk in cooler. Must have a thermometer in all coolers that hold potentially hazardous food to ensure adequate temp is maintained.
- Smoke free air act
Do not have official no smoking sign posted on entrance. Must post official no smoking sign at entrance of store.
- Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
Observed raw meats stored above ready to eat food in retail display case. Must adequately separate raw animal products from ready to eat food by storing ready to eat food above raw products.

Lights in dry storage (back) area do not have adequate covers or shatter resistant bulbs. Must provide shatter resistant bulbs or adequate covers to prevent contamination.
