Observation:Observed mold buildup on sides of ice machine.

Observation:Observed containers of shell eggs stored on walk-in cooler shelf above salad dressings, and bags of carrots and broccoli stored in containers with raw fish and raw ground beef. Moved, corrected on site. Repeat violation.

Observation:There is no handwashing sign in men's restroom, a sign was printed.
- Food storage - preventing contamination from the premises
Observation:Observed box of cheeses on walk-in cooler floor. Repeat violation.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation:Observed containers of chicken, potatoes, coleslaw, baked beans, hamburgers, and green beans with no dates on the containers.
- Manual warewashing sinks requirements
Observation:There is only a two-compartment sink in the kitchen, sanitizing is performed in a separate plastic tub. Repeat violation.
