Observation:Observed employee washing hands in three-compartment sink - the water to the handwashing sink was turned off. Corrected on site.

Observation:Observed bags of ice are not labeled with name and address of source, and one larger package of ham loaf is not labeled with store name and address of list of ingredients.

Observation:Food establishment license expired May 24, 2014 and has not been renewed. Owner given renewal form.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation:Observed buildup of soil and food particles on table that holds the meat slicer and sides of equipment in meat-cutting room.
- Indoor and outdoor surfaces
Observation:Observed missing floor tiles and exposed wood subfloor in meat-cutting room and store aisles - flooring no longer smooth, durable, and easily cleanable.
- Material characteristics of non-food contact surfaces
Observation:Observed wood chest of drawers is used for storage of ingredients and equipment in meat-cutting room next to meat slicer - varnish is deteriorating on the outside of the drawers, no longer smooth, nonporous and easily cleanable.
