Certified Food Protection Manager with management and supervisory responsibility employed by the establishment Observation:facility does not yet have a certified manager...please identify when this will be attended and submit certificate information upon completion. Physical facilities maintained clean: Cleaning frequency, restrictions, and methods Observation:Cleaning frequency appears to have fallen...frequency will need to be increased until back into compliance. (build up on floors, by walls, around equipment...cobwebs and dust bunnies) 6-501.11 Repairing. PHYSICAL FACILITIES shall be maintained in good repair. 6-501.12 Cleaning, Frequency and Restrictions.(A) PHYSICAL FACILITIES shall be cleaned as often as necessary to keep them clean. Physical facilities maintained in good repair Observation:Some areas of the physical structure are in need of repair. Ie- broken tiles behind counter, through walkway from kitchen to service area and out in seating area.Freezer floor is broken. Back door does not site flush and is difficult to close, the scoop holder is chipped up, soap dispenser is broken from wall in ladies restroom. Cutting surfaces maintained Observation:cutting boards are grooved, rough and darkened. These need to be replaced. Plumbing system maintained in good repair Observation:Urinal is broken. Plastic wrap is around faucet at warewash...remove this. If it was in place there due to a leak, then it will need to be repaired.	08/19/2015	Regular
