- Labeling
Various spices received in bulk and repackaged on site are not labeled correctly. Observations related to improper labelling include the following: 1) The name and address of the packer were omitted from labels that were found on products, 2) The common name of foods declared on the food label are only included in Espanol. For example, ground cumin was labelled as "Comino Molido". Photographs of improperly labelled products were taken.

- Screening
Failure to provide adequate screening or other protection against pests. Specifically, On 7/20/2015 I observed an unsealed and unscreened gaped opening along the top of the large overhead door in the main warehouse. Reinspection observations of 7/22/2015: I observed no change in the way the overhead door in the main warehouse is sealed for the control and potential entry of pests. Additionally, I observed the overhead door in the room adjacent to the main warehouse open upon arrival and left open for over an hour. Reinspection observations of 8/31/2015: Not only is there still a significant gap along the top of the main outside overhead warehouse door, but the outside overhead door in the room adjacent to the main warehouse was open upon arrival and wasn't closed until requested by inspector. At least two birds were observed in the warehouse and bird-like droppings were observed throughout the warehouse. Bird-like droppings were observed on pallets, boxed foods such as Yellow Corn Chips, and on exposed 9" Round Handi-Foil Pans, to name a few for example. Other food products that were openly stored and exposed to the environment include Chile Peppers, Laurel Leaves, ground & whole Cumin, and Paprika Powder. Reinspection observations of 10/08/2015: Repairs have been made to the top of the main outside overhead warehouse door to prevent the entry of birds. Gaps along the bottom and sides of all outside warehouse doors were discussed as needing improvement for controlling insects, rodents and birds. Bird-like droppings that were referenced in the 8/31/2015 inspection report are still evident.

Lack of effective hand cleaning preparations. Specifically, I observed the handwash sink located in the plant nonfunctional due to not being fastened to the wall and appears to be held up byway of the drain pipe with no support. Reinspection Observation on 7/20/2015: I observed no improvements made to the wall mounting of the handwash sink located in the processing area. Additionally, there were no hand towels available at the handwash sink located in the processing area, nor the handwash sink located in the toilet room.
- Drip and condensate
The plant is not constructed in such a manner as to prevent condensate drip from contaminating food. Specifically, I observed condensate dripping from the condenser unit in the walk-in cooler that was landing on and soaking boxes of whole tomatoes. Additionally, a large amount of ice that appears to have originated from dripping water from the condenser in the walk-in freezer was observed built up on boxed foods and on the floor. Reinspection observations of 7/20/15: There was some improvement in the amount of condensate drip from the condenser in the walk-in cooler and the some ice has been removed from the floor of the walk-in freezer. Company management stated that adjustments are being made to both refrigeration units in an effort for correction.

Specifically, Specifically, I observed a large quantity of rotting fruit and vegetable waste in a pile on the ground just outside the plant walk through door, in addition to all around the dumpster. Additionally, juices from the rotting waste was observed flowing from both areas and attracting a great number of insects. A foul odor is also present throughout the area between the plant and dumpster. Reinspection observations of 7/20/15: I observed no reduction in the amount of waste that is currently piled in and around the outside dumpster awaiting pickup. Garbage pickup is said to be scheduled for Wednesday, 7/22/2015.

Failure to maintain toilet facilities in a sanitary condition. Specifically, I observed spent tissue all about the toilet room floor. Toilet room also lacked hand drying and the door was propped partially open.

- Odor, attractant for pests, harborage
The storage and disposal of rubbish does not minimize the development of odor and potential for waste becoming an attractant and harborage or breeding place for pests. Specifically, I observed a large amount of waste and a foul odor in the area where repackaging of raw ingredients takes place in the plant, in addition to a large quantity of rotting foods and foul odor in the center of the warehouse where Avocados, Peppers, Oranges and chicken breasts are piled up and covered with a great number of insects resembling flies and gnats. Warehouse temperature at the time of inspection was 80 degrees.
- Manufacturing conditions
Failure to package and store foods under conditions and controls necessary to minimize the potential for growth of microorganisms and contamination. Specifically, I observed open bags of raw ingredients next to an overflowing waste barrel at the repackaging area of the plant that had a great number of insects resembling flies and gnats in the immediate area.
- Cleaning and sanitizing operations
Failure to conduct cleaning and sanitizing operations for utensils and equipment in a manner that protects against contamination of food-contact surfaces. Specifically, I observed the three-compartment ware-wash sink heavily soiled and containing foreign material that resembled garbage.

- Failure to clean - general
Failure to clean food-contact surfaces as frequently as necessary to protect against contamination of food. Specifically, the three-compartment warewash sink was observed heavily soiled.
- Self-closing doors
Toilet facilities lack self-closing doors. Specifically, main toilet facility lacks a self-closure on the door. Observed overflowing waste receptacle in the toilet facility.
- Holding in bulk or suitable containers
Failure to hold ingredients in bulk or in suitable containers so as to protect against contamination. Specifically, large work in progress containers of ingredients were observed stored open and must be protected from contamination between uses.
- Labeling
Labeling: repackaged containers of spices and other ingredients shall be labeled with the name and address of this facility, the common name of food packaged, a list of ingredients if the contents contain two ingredients or more, any cautionary statement such as allergens, and the net weight. All information shall be displayed in English. If a second language is desired, it must be in addition to English.
- Precautions against contamination--micro, foreign substances
Failure to take necessary precautions to protect against contamination of food with microorganisms and/or foreign substances. Specifically, condensate drip has frozen and building up on boxed food storage in the walk-in freezer. Observed bags of onions stored on the floor in dry storage area of warehouse.
- Hand cleaning and sanitizing preparations
Lack of effective hand cleaning preparations. Specifically, the handwash sink located in the warehouse is inoperable at the time of inspection.
- Screening
Failure to provide adequate screening or other protection against pests. Specifically, gaps all around both overhead doors allow for the entry of rodents and pests - two birds were observed in the warehouse at the time of inspection and were evacuated while onsite.
