    The presence of insects rodents, and other pests are controlled as required
  • Cleanability of floors, walls, and ceilings
    Observation: The walls and ceiling in the dough prep area are of porous drywall along with exposed raw wood that are not easily cleanable. There are some missing floor tiles in the warewash area that is allowing raw wood to be exposed.
  • A temperature measuring device with a suitable small diameter probe is provided and readily accessible to measure the temperature of thin foods
    Observation: There is no thin tip digital thermometer to check the temperatures of thin foods.
  • Bare hand contact with ready to eat foods
    Observation: There was bare hand contact with baked garlic bread. Corrected on site.
  • Manual and mechanical warewashing equipment, chemical sanitation - temp, pH, concentration, and hardness
    Observation: There was no detectable sanitizer on the test strip for the warewash machine. Corrected on site by replacing the container of sanitizer.

  • Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
    Only one salad was in a container labeled with date of preparation, other salads and pastas were not in date-marked containers.
  • Roasts held at a temperature of 130°F or above
    Salads in salad bar were in containers on top of ice, potato salad was 49 degrees. Ice was added around containers, corrected.
  • Cleaning frequency of food contact surfaces (PHF/TCS)
    Observed food particles on meat slicer - person in charge does not know how to take machine apart and did not use the machine today, the operator did not clean and sanitize the slicer before leaving for the day. Repeat violation. Observed soiled glasses, pizza pan and butter brush in basement near dough room. Observed ice scoop on top of dusty ice machine in basement. Observed mold and old dough in white plastic tub in basement, lid of tub has heavy soil deposits.

    There are exposed studs in basement dough room walls and ceiling, drywall is not painted to make smooth, nonporous and easily cleanable - repeat. Observed missing tile on dishroom floor under the three-compartment sink, exposing the wood subflooring.

    Observed open drinking glasses by ice bin at front counter - repeat violation, corrected.

    Observed home-canned jar of pepper jelly on kitchen shelf - removed, corrected.

    Observed heavy flour deposits on floors, equipment and walls in basement dough room, especially between the drywall and the studs. Repeat violation. Observed bacteria and mold/yeast deposits on floor by north cement block wall, where water is seeping in - the deposits are quite significant and cover a wide area.
