    Observation: One side of lower level of produce cooler with stored grapes, paprika, produce did not work properly and temped between 46-55*F. A pudding, a cheesecake and a fruit cake stored on that side discarded at the time of inspection. The manager stated that were displaced there. All potentially hazardous (PHF/TCS) foods must be maintained at 41°F or below. Call for service or do not keep PHF for (time/ temperature control for safety) food there.
  • Light intensity
    Observation: Missing light or bulb broken in walk-in cooler. Must provide adequate light intensity in the area where is food stored inside reach-in refrigerators for easily cleaning and maintaining.Corrected on site.
  • Food temperature measuring devices are provided and readily accessible
    Observation: Missing thermometers in most reach-in coolers such as, ice cream, milk, creamer cooler etc. Must provide the thermometers in all coolers to ensure adequate temp of food is maintained at least 41*or below. Provide thermometers.
  • Food temperature measuring devices are accurate to ±1°C or ±2°F in the intended range of use
    Observation: A digital thermometer did not show correct temperature in the produce cooler, it went up and down between 35-46.5*F. Food temperature measuring device scaled in Fahrenheit shall be accurate to ±1°C or ±2°F. Adjust the thermometer or provide one that measure temperature correctly.
  • Food storage - preventing contamination from the premises
    Observation: Food such as coffee stored on the floor in the middle of the store on carton. All food must be stored above the floor on case lot handling equipment. Corrected on site.
  • Storage or display of food in contact with water or ice
    Observation: Excessive ice buildup in all chest freezers in the store. Must defrost to prevent source of contamination and keep freezer free of ice. Clean as needed.
  • Equipment location, installation, repair, and adjustment
    Observation: Both gasket on ice reach-in cooler were detached and in poor repair. All equipment must be maintained in accordance with manufacturer requirements. Replace as needed.
  • Package integrity
    Observation: Several dented cans of food including baby food found stored on the shelves. May not use any can that is dented on top, bottom, or side seals. Dented can stored on dry goods shelf. Removed and corrected on site.
  • Established procedures for responding to vomiting and diarrheal events
    Observation: No written procedure for Norvirus events. Food establishment must have procedure for employees to follow when responding to vomiting and diarrheal events within the food establishment to reduce foodborne disease transmission. Information regarding the procedure given to Manager.

    Observation: The last inspection report is not posted for consumers to view. Must post the inspection report at eye level for consumers to view.

  • Handwashing sink equipped to provide water at a temperature of at least 100° F through a mixing valve or combination faucet.
    Hand sinks in both women and men's restroom not equipped to provide 100 degree water through a mixing valve or combination faucet. Must provide 100*F water for handwashing sink.
  • Roasts held at a temperature of 130°F or above
    1) Hy-Vee ham steaks in dairy and ready to eat meat reach-in cooler was temped at 45*F. Items discarded 2) Several items at the front of the stock in reach-in cooler near copy machine were not temped at 41*F or below (Chinese pre-packed food 45*F, potato salads 50*F, and hot dog 47*F). The cooler is working, and the thermometer was showing the temp at 39.6*F. Items diiscarded, and keep watching all foods temperature in this cooler.

    Palermo's Pizza reach-in freezer has ice buildup. Manager said freezer defrosted every two weeks. Must defrost freezer more frequently to prevent contamination.

    1) Pringles potatoe chips in storage 2) Fruit snacks near customer service were stored on the floor. All foods must be 6" off the floor.
