Observation: Several flooring tiles in walk-in cooler room are cracked. Replace as needed.

Observation: Paper towel dispenser in kitchen was empty. Refilled. Must provide paper towels for hands drying.

Observation: Sanitizer bucket in bar is not labeled. Must label all working container to easy identify. Corrected at time of inspection.
- Bare hand contact with ready to eat foods
Observation: Employee prepare ready to eat food with bare hands. Must wear gloves or use utensil when preparing RTE food to prevent contamination.
- Food temperature measuring devices are provided and readily accessible
Observation: Thermometer not provided in reach-in cooler. Must have thermometer to ensure adequate temp is maintained. Corrected at time of inspection.
- Raw animal foods separated from each other during storage, separation, holding, and display
Observation: Raw eggs were stored above ground beef in reach-in cooler. Raw animal food must be stored by the cook temperature: higher cook temp items must be stored on the lower shelf.
- Multiuse food contact surfaces are constructed of safe materials
Observation: Food cardboard boxes were reused for other than their original products in reach-in cooler. Must provide smooth and cleanable container for this purpose. Removed all cardboard boxes during the inspection.
- Material characteristics of non-food contact surfaces
Observation: Inside of the cabinets in bar area have bare wood surfaces. Must varnish or paint with light color at least two coats to previde smooth and easily cleanable surface.
- Sanitization methods - hot water, chemical
Observation: QA sanitizer in the bar three compartment sink was tested low. QA sanitizer concentration must be maintained between 200 to 400ppm. Corrected at time of inspection.
- Eating, drinking, or using tobacco
Observation: Observed employee beverage cups have no lid in both bar and kitchen. Food employee must drink from a closed beverage container to prevent mouth to hand contamination.
- Test kit provided and used to measure sanitizing solution concentration
Observation: Didn't have QA test strips at time of inspection. Must provide QA test strips to ensure concentration is maintained between 200 to 400ppm.
- Outer openings are protected
Observation: 1) Didn't install self-closing device on exit door in kitchen. All exit door must be install self-closing device. 2) Exit door in dinning room has light showing around outer edges. Replace new door sweep to prevent entry pest.
- Utensils and pressure measuring devices maintained
Observation: Observed a burned popcorn scoop. All utensils must be maintained in good condition. Replace.
- Handwashing signage
Observation: Handwashing sign not posted in Men's restroom. Every hand sink to be used by employee must has sign posted reminding employee to wash their hands.
