- Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
Observation: Employee health forms have not been filled out for all employees, and only have Form 1B, newest signed document was 2009 of those sampled. All employees must fill out form 1A and 1B, guidance documents provided.
- A temperature measuring device with a suitable small diameter probe is provided and readily accessible to measure the temperature of thin foods
Observation: Staff states no thermometer at facility to check hot holding or cold holding food temperatures. Must provide thin diameter food thermometer.
- Test kit provided and used to measure sanitizing solution concentration
Observation: No sanitizer test strips available. No sanitizer being used for warewashing.
- PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
Observation: Walk in cooler Temp of 44 degrees at 3:10pm, 49-50 degrees at 4:03pm. Cooler storing breakfast sandwiches that have been thawing for over 24 hours, temping 44 degrees. --Observation: Reach in cooler of ready to eat deli sandwiches temping 45-46 degrees, thermometer in unit reading 44 degrees. Unit must be posted not for sale and sandwiches must be discarded until fixed.
- Drying mops
Observation: Mop stored in mop bucket. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment or supplies.
- Handwashing signage
Observation: No hand wash sign at sink in bathroom.
- Certified Food Protection Manager with management and supervisory responsibility employed by the establishment
Observation: Facility must have certified food protection manager on staff by 7/20/16.
- Manual and mechanical warewashing equipment, chemical sanitation - temp, pH, concentration, and hardness
Observation: Staff states 3 compartment sink not used for ware washing of cappucino machine cups or coffee pot. No sanitizer is being used. This is a repeat violation. All food contact surfaces must be sanitized immediately and then on a regular basis as designated by equipment.
