    Observation: Chicken is being partially cooked and reheated at a later time. Establishment has no written procedures and has not obtained regulatory approval for this process.
  • Multiuse food contact surfaces are constructed of materials that are: durable, corrosion-resistant, non absorbent
    can withstand repeated warewashing
  • Established procedures for responding to vomiting and diarrheal events
    Observation: No procedure in place. Emailing information to implement.
  • Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
    Observation: Raw meats stored above cooked foods and precooked meats in the walk in cooler.
  • PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
    Observation: Container of ham in make line holding at 47*. Moved container of ham to walk in cooler.
  • Handwashing aides and devices, use restrictions
    Observation: Handwashing sinks in the bar area and kitchen prep line are not accessible for handwashing. Sinks are unclean and used for other purposes other than hands.
  • Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
    Observation: No verifiable policy of reportable illnessess. E-mailing information to implement.
  • Discarding ready to eat, PHF/TCS food prepared on-site or opened commercial container held at 41°F for 7 days
    Observation: Several cooked foods not marked with the preparation date. Items were datemarked at time of inspection.
  • Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
    Observation: All coolers shall have thermometers inside or outside to monitor temperatures.
  • Single -service and single-use articles may not be reused
    Observation: Single use trays are being reused for french fry transfer.

    Observation: Observed a water drip from the ceiling of the soda concentrate storage room that is coming from snow melt on the roof. Maintenance has requested repairs made and roofers are waiting for favorable weather conditions.

    Observation: Florescent lights located above prep areas throughout the kitchen are required to have effective shields.
  • Physical facilities maintained in good repair
    Observation: The deteriorated lower wall damage in the warewash area has not been scheduled for repair yet, but will be.

    Observation: Observed some missing end-caps for the protective shields on the florescent lights located above food prep areas in kitchen.
  • Physical facilities maintained clean: Cleaning frequency, restrictions, and methods
    Observation: A thorough cleaning, is needed throughout the kitchen and dry storage areas, including hard to reach areas such as under shelving and behind equipment. Observed soiled carpeted areas under booths in the dining area.
  • Outer openings are protected
    Observation: Observed weather stripping missing along the bottom of the double doors referred to as the daily truck delivery doors - new weather stripping was installed while onsite.

    Observation: Observed soiled meat slicer - corrected onsite. All equipment that comes into contact with foods that are temperature controlled for safety shall be cleaned at least every four-hours, or stored at required temperatures. Observed slightly soiled ice maker, in addition to soiled ice scoop that was improperly stored on a soiled surface on top of ice maker.
  • Plumbing system maintained in good repair
    Observation: Observed partially clogged floor drain under ware wash machine.
  • Pre-cleaning equipment and utensils and loading soiled items into warewashing machines
    Observation: Observed excessive food residue in the screens of warewash machine - Garbage disposal unit not working at the time of inspection.
  • Handwashing cleanser, availability
    Observation: No hand soap provided at the hand wash sinks in the kitchen, food prep areas and bar.
  • Premises maintained free of insects, rodents, and other pests
    The presence of insects rodents, and other pests are controlled as required
  • Physical facilities maintained in good repair
    Observation: Observed wet and deteriorated lower wall damage in the ware-wash area.
  • Cleaning of cooking and baking equipment
    Observation: Observed heavy black apparent char and/or grease buildup on the exterior of cookware that is leaving deposits on prep-table cutting boards and curtains located on the exit end of the warewash machine.
  • Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
    Observation: Observed stagnant water and food residue in stored Banquet warming pans and serving trays.
  • Protecting food from environmental contamination
    Observation: Observed a portable bin of flour, soiled, with linens stacked on top near the washer/dryer area - Flour was disposed of. Observed drip from soiled wet floor-mats that were folded and stacked on shelving in the storage room where soda fountain concentrate is located. --Observation: Observed mold on the ceiling and portions of the upper walls in the walk-in cooler located in the bar area, in addition to a slight mold accumulation on the ceiling above the warewash area in the kitchen. Observed peeling paint on the ceiling of the walk-in freezer. Observed areas throughout the kitchen, food prep and storage areas where the ceiling tile were missing, displaced or stained from apparent roof leakage - no wet tile or leakage was observed.
