    Observation: Food employee have no hair restraint during the food preparation ( frying chicken) in the kitchen and serving.
  • Food properly labeled with major food allergens
    Observation: Self service foods (donuts,cupcakes and other cookies are not properly labeled with list of ingredients or with major food allergens. It does not have the large sticker posted on the case stating the ingredients in the pastries.
  • Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
    Observation: 1) Boxes of raw shell eggs, raw fish and raw chicken stored above ready-to-eat food (roasted chicken) in the kitchen cooler. Removed and corrected. 2) Raw chicken and raw shell eggs stored above green salad in walk-in cooler, placed below and corrected. This is a repeating violation. Provided educational material.
  • Food is properly labeled
    Observation: Store packed ice for sale in the bags.The ice bags have no store identification number labeled in reach in-cooler for sale.
  • Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
    Observation: Store has forms with no employees signatures.Food employees have not read and signed the Food Employee Illnesses Reporting Agreement.

    Observation:Several small containers containing coleslaw and chopped pineapple were dated 1/30/16 (past shelf due 7 days).Discarded and corrected.

    Observation:Bulk containers with sugar and flour are not identified with common name in dry storage area.
  • Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
    Observation: Several food containers were not date marked at time of inspection: coleslaw, tuna salad, chicken salad, taco dip and open ham in- reach kitchen cooler. Date marked and corrected. This is a repeating violation.
  • Food storage - preventing contamination from the premises
    Observation: Several boxes of food stored on the floor in walk-in freezer. This is a repeating violation.

    Observation: Mold build up in ice machine. This is a repeating violation.
  • Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
    Observation: Open air-cooler in retail area (with sandwiches) has no thermometer. This is a repeating violation.

    Observation: 1) Boxes of single use items (to-go-boxes) being stored on the floor behind the counter. Removed and corrected. 2) Cups and to-go boxes) are being stored on the floor in dry storage basement area. This is repeating violation.

    Observation: Ice machine in the basement has mold buildup and must be cleaned.
  • Miscellaneous sources of contamination
    Observation: Observed box of potatoes stored adjacent to the hand sink in the kitchen. Must move the box of potatoes to prevent contamination.
  • Food temperature measuring devices are provided and readily accessible
    Observation: Chesters chicken warmer and True 2 door prep cooler both do not have a thermometer inside. Must have a thermometer in all cold and hot holding equipment.
  • Food storage - preventing contamination from the premises
    Observation: Food was observed stored on the floor in the walk in freezer. Must store all food at least 6 inches off the floor.
  • Storage of clean linens, single-service, and single use articles
    Observation: Observed single use items stored on the floor of the basement. Must store all food/food items at least 6 inches off the floor to prevent contamination.
  • Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
    Observation: Observed some items that were not date marked at time of inspection (cheese, pizza toppings, baked potatoes, etc.). Must date mark all ready to eat food with date opened/prepared and then discard within 7 days. Also was using manufacturer date for deli salads as a method of safety. Manufacturer date is only for quality, and container must be discarded 7 days after opening. Items were remarked.

    Observation: Self service items (donuts and cold items in the take & bake cooler) are not properly labeled. Items must have at a minimum: the common name of the food, list of ingredients in order of predominance by weight, net contents, name, and place of business.

    Observation: Eggs and pork (raw) were observed stored above the ready to eat baked potatoes in the three door stand up cooler in the kitchen. Must store all ready to eat food above raw products to prevent contamination.
