Observation: Food debris buildup at inside of reach-in freezer. Clean as needed.

Observation: Sanitizer at all locations were tested lower than 100ppm. Must replace sanitizer more frequently to ensure concentration is maintained at 200-400ppm. Replaced. Corrected at time of inspection.
- Food storage - preventing contamination from the premises
Observation: A box of food was stored on the floor in walk-in freezer. Food must be stored at least 6" off floor to prevent contamination.
- Handwashing signage
Observation: Handwashing signs not posted at both hand sink in kitchen. Every hand sink to be used by employee must has sign posted reminding employees to wash their hands. REPEAT VIOLATION.
- Equipment location, installation, repair, and adjustment
Observation: The left bottom of chute in ice machine is cracked. Replace. Food equipment must be maintained in good condition.
- Food temperature measuring devices are provided and readily accessible
Observation: Must provide thermometer in each cold hold unit to ensure adequate temperature is maintained. Put thermometer closer to the door area.
- Plumbing system maintained in good repair
Observation: Hot water not provided at all sink except three compartment. Must provide hot water during all business hours. Called for service during inspection.

- Plumbing system repaired according to law
Observation: 1) Mop sink drain slow. Service was here during inspection. 2)Faucet on 3 compartment sink is leaking 3) Handle (hot water side) on hand sink next to 3 compartment sink is leaking. Called for service. All plumbing must be maintained in good condition.

Observation: Food were stored on floor in walk-in cooler. All food must be 6" off the floor to prevent contamination and cleaning purpose. Corrected at time of inspection.
- PHF/TCS food that is cooked and cooled on premises is reheated within 2 hours to 165°F or above for 15 seconds for hot holding
Observation: Two large containers of chili soup were not reheated to 165*F for hot holding. The center areas were temped between 108*F to 112*F when it was put in service warmer for hot holding. Reheated again. Corrected at time of inspection. MUST TRAINING FOOD EMPLOYEE HOW TO REHEAT FOOD AND HOW TO USE THERMOMETER TO ENSURE FOOD TEMPERATURE.
- Cleaning frequency of food contact surfaces (PHF/TCS)
Observation: A small amount of mold is on the front side ice chute in ice machine. Must clean more frequently to prevent contamination.
- Storage of clean equipment and utensils
Observation: Single service items were stored on floor in dry storage. All single service items and utensils must be stored 6" off floor. Corrected at time of inspection.
- Equipment location, installation, repair, and adjustment
Observation: Ice buildup in walk-in freezer. Repair as needed to ensure equipment stay in good condition. Clean more frequently to prevent contamination.
- Material characteristics of non-food contact surfaces
Observation: Cardboard were reused underneath prep table in kitchen. Cardboard is not a smooth, easily cleanable, ad nonabsorbent surface.
