    Observation: 1) Visible buildup on can opener slicer, 2) Food debris buildup the inside of microwave in kitchen. This is a repeat violation.

    Observation:1)Three compartment sink have leaks on faucets. 2) Atmospheric vacuum breaker cap was broken.

    Observation: Packaged food stored in both freezers that have excessive amount of ice buildup in dry storage area.

    Observation: Both front prep coolers have torn door gaskets.

    Observation: No handwashing sing posted in women's restroom.This is a repeat violation. A sign provided,corrected.

    Observation: Several squeeze bottles with oils were not labeled in the different places in the kitchen. Pointed out to Manager the same colour of detergent that was not labeled. Manager labeled them and corrected on site.

    Observation: No procedures for responding to vomiting and diarrheal events. Provided to post on the wall. Corrected.

    Observation: Two plastic spatulas damaged and are no longer cleanable.

    Observation: Boxes of single use items pizza (to-go boxes) are being stored on the floor next to the pizza owen.

    Observation: No chlorine test strips provided. Must have chlorine test kit to check dish machine solution concentration.

    Observation: Found dented can on can rack. Must discard any can that is dented on top, bottom, or side seals. Also any can that cannot sit level on shelf without wobbling.

    Observation: 1)Several bags of tortilla were stored on the floor in garage. 2)Food stored on the floor in walk-in cooler. Food must be stored at least 6" off the floor to prevent contamination and cleaning purposes.

    Observation: Gaps under or around exterior door at garage . Provided door sweep to close gap and prevent pest entry into facility. REPEAT VIOLATION

    Observation: Used dishes were stored into hand sink in kitchen. Hand sink is only used for hands washing. Must have sink clear to ensure employee are washing when necessary.

    Observation: Didn't provide thermometer into the following coolers: 1)Prep cooler next to pizza oven, 2) Draft beer station (Ready to eat foods were stored) 3)Prep cooler next to the register. Must put thermometer at the warmest area in cooler to ensure adequate temperature is maintained.

    Observation: Taco meat container were not date marketed in walk-in cooler. Must date mark all potentially hazardous food if not used within 24hrs after opened or preparation.

    Observation: Light bulbs were not covered or shielded in pizza hot holding equipment. Must provide an adequate cover or shatter resistant bulbs in area where there is exposed food.

    Observation: Visible buildup on can opener. Clean more frequently to prevent contamination.

A higher concentration makes solution toxic. Use test strips frequently to ensure adequate concentration. REPEAT VIOLATION.
