- KDA_5_20515B - --There was a sewer gas odor in the west doorway into the kitchen.
- KDA_6_50111 - --The wall board behind the 3-vat sink was bubbled and peeling. The ceiling covering was loose and falling over the west clean dish storage shelf by the 3-vat sink.

- KDA_7_20111A - --A bottle of YES degreaser was stored next to a clean stainless steel lids on the clean dish shelf by the 3-vat sink. (COS-degreaser was moved).
- KDA_7_20612 - --There were two open boxes of Decon rodent poison in the water heater room in the kitchen. One container was north of the water heater, and one container was by the west door.

The ranch dressing had just been prepared, as per owner. Walk in freezer: rice 68 F. The rice had been cooked then placed in the freezer to cool two hours prior, as per owner.
