  • *4.4 Observed the spray bottle with QAC sanitizer was 400 ppm or greater. Maintain QAC sanitizer at 200-400 ppm per manufacturer specifications.
  • *3.4 Observed the chicken wings and boneless wings were lacking a date mark. Ready-to-eat foods shall e dated the day of preparation and shall be used or discarded in seven calendar days.
  • *4.5/4.6/4.4 The observed the cutting board was scored. Food contact surface shall be washed, rinsed, sanitized and shall be clean to sight and touch. Repair or replace cutting board
  • 4.5/6.4 Clean the following areas:
  • R/- Dusty hood filters under both hood units.
  • - Interior of the microwave
  • - Catch trays for the pizza oven
  • - Repair the damaged handle for the microwave
  • R/- Dusty gas line and pipes attached to the oven
  • - Shelves holding pizza pans
  • - Container with the test kits
  • R/- Floor/wall junctures throughout are stained or soiled
  • 6.2 Replace the lacking light shied above the pizza oven to prevent contamination of food.

  • R/*3.4/4.4 The internal temperature of the case holding salads was 45°F, chicken was holding at 48°F, and 51°F in the reachinunit, and the cheese and the tomatos on the salad in the case were holding at 47°F. Refrigeration units and foods shall be maintained at 41°F and below.
  • *7.0 Label the clear liquid spray bottle, with the name of the chemical.
  • *5.1/2.4 Provide papertowels in the restroom, to facilitate with proper hand washing.
  • R/6.4 Clean the floor/wall junctures under the hand sink, which were soiled.
  • 6.1 Provide lacking light shield in the front area, to prevent contamination.
  • 6.2 Repair the non-functioning light fixture by the Pepsi unit, above the make table and three vat sink.
  • 4.5 Clean the exterior of the lines running to the oven, condenser fan cover, hood filters under the hood, which are dusty.
  • 6.4 Repair the leaking ceiling tile above the walk-in cooler.

  • *4.4 Jojo slicer was soiled. Food contact surface shall be washed, rinsed, and sanitized, and shall be clean to sight and touch.
  • *4.4 Observed the spray bottle labeled sanitizer contained greater than 200 ppm. Maintain QAC sanitizer at 200 ppm.
  • *3.4 Observed chicken wings dated 11/7. Food shall be dated the day of preparation and shall be used or discarded in seven calendar days. Properly date this item.
  • R/4.5 Clean the bulk containers which are soiled, the wired racks holding pizza pans is soiled, shelf under the dough table is soiled, and top of the oven is dusty.
  • 6.4 (R)Clean the floor/wall junctures throughout as needed, which were soiled.
  • R/4.1 Provide accurate thermometers in the salad display case to assure 41°F is maintained. Thermometer was provided today.
  • R/6.4 Fryer, potato wedger, and True reach-in refrigeration unit was not in use during this inspection. Remove unused unnecessary equipment from premises.
  • 3.2 Observed water spray bottle hanging from the garbage can. Store spray bottle in a means to prevent contamination.
  • 6.1 Provide lacking light shield, to prevent contamination.
  • 4.0 Observed a discolored towel under the oil container. Provide a cleanable surface.

  • Operation added Anvil warmer unit, model number FMA 7036 with ETL commercial label.

  • *3.4 Chicken wings were holding at 43°F, shredded cheese was holding at 42°F and the internal temperature of this case was holding at 40-44°F, the salad case contained an internal temperature of 44°F. Food and refrigeration units shall be maintained at
  • 41°F and below.
  • *3.4 Ham was lacking date marking. Food shall be dated when removed from the freezer and shall be used or discarded in seven calendar days.
  • *7.1 Observed motor oil stored above pizza pans in the rear storage area. Store toxic chemicals in a separate designated area from food and equipment.
  • 4.5 Clean the salt container which is soiled, interior of the Maxium freezer and door seals contains ice accumulation, the wired racks holding pizza pans and wing sauces are soiled, interior and door seals on the pizza reach-in unit, door seal on the walk-in cooler, and both hood units are dusty.
  • R/3.2 Label the bulk container containing corn meal, with the name of the food.
  • 6.4 (R)Clean the floor wall junctures and floor under equipment throughout as needed, floor drain and floor surface surrounding the floor drain of the preparation sink, and (R)the wall behind the dough sheeter, which were soiled.
  • 4.1 Provide accurate thermometers in the salad display case and reach-in refrigerator with chicken wings, to assure 41°F is maintained.
  • 6.4 Fryer, potato wedger, and True reach-in refrigeration unit was not in use during this inspection. Remove unused unnecessary equipment from premises. Additionally clean these pieces of equipment.
  • Note: Reportedly microwave is for personal use only.
  • Operation pizza oven was in disrepair during this inspection.
