- R/C/*3.4 Observed the following raw animal foods stored improperly:
- -raw shell eggs stored on top of hot dogs in the walk-in cooler
- -Bob Evans raw ground sausage stored above ready-to-eat brats in the retail cooler
- Store raw animal foods separate from and below ready-to-eat foods in order to prevent possible cross-contamination.
- R/6.2 Repair the non-functioning light in the walk-in cooler.
- 6.2 Replace the 3 missing lights in the Skinny Cow/Drumsticks freezers in order to provide proper lighting.
- 4.4 Observed ice build-up on the inside of the 3 Mayfield ice cream freezers. Also, observed condensation on the outside of the freezers. Make any necessary repairs and keep food protected from possible contamination.
- NOTE: Observed Slim Fast chocolate mint bars with an expiration date of March 3, 2012 on the retail shelf. Check and rotate stock.
- -Observed proper chemical storage.
- -Observed baby food/formula properly within expiration dates.
- -Ambient air temperatures of coolers were properly below 41°F.

- *901:3-4-15 The following baby foods items were expired:
- - Gerber vegetable and chicken dinner was dated 2/5/11
- Baby food items shall be checked rotated and removed from normal stock by the expirations date.
- *7.1/7.2 Observed Sani-Hands and Sani-Hands kids was stored above children's plates, cups, bottles, and bowls, also coffee maker cleaner was stored with and above equipment. Toxic chemicals shall be stored in a separate designated area separate from food and equipment to prevent contamination.
- *3.4 Observed ready-to eat polish sausage was stored with raw eggs and bacon, also raw sausage was stored with other ready-to eat food items. Different types of raw animal foods shall be stored separate from or not with ready-to-eat foods to prevent crosscontamination.
- 3.5 Observed the expiration dates on the Beech-nut chicken with broth, and Beech-nut beef with broth were covered. Relocate price tags.
- 4.4 Observed three damaged door seals with the Gatorade, Heineke, and Seagrams products. Repair this area to provide a cleanable surface.
- 4.8 Store bagged snacks six inches off the floor to facilitate cleaning and to prevent contamination.
- 6.2 Repair the non-functioning light fixture in the walk-in cooler.
