This visit is in reference to RFS 13-0026 regarding a dog in the establishment. At the time of inspection there was no dog on the premise. After speaking with an employee it was discussed that there was a dog in the establishment and a worker brought the dog to work today. I spoke with the assistant manager over the phone and let her know about the complaint and that having a dog in the establishment is prohibited.
Complaint closed.
No violation noted during this evaluation.
1/31/2013Complaint Investigation
This inspection is in reference to complaint 13-0001 regarding the creamer at the self-serve area being warm. At the time of inspection the creamer was at 33 degrees F. I talked to worker about a policy regarding temperature control to ensure the temperature remains below 41 degrees F. Complaint closed.

HACCP: Discussed sanitizer concentration. Boil order HACCP handout discussed.

  • Food protection during storage, prepartion, display, service, transportation
    Boxes of coffee are being stored on the floor.
    Containers of food shall be stored a minimum of six inches above the floor or on easily moveable dollies, racks or pallets to protect the food from splash and other contamination, and to permit easy cleaning of the storage area
  • In-use food dispensing utensils properly stored.
    Handles of scoops were in the following product: coffee beans and ice.
    Store food dispensing utensils with the handles out of the product.
  • Critical: Hands washed, good hygienic practices (observed).
    Worker is not washing hands after handling money and before serving drinks or using deli tissue.
    Hands shall be washed thoroughly before using utensils, including: deli tissues, spatulas, tongs or single-use gloves.

    The following food contact surfaces are soiled: 1. baffles of both ice machines, 2. inside the upright freezer in the dry storage area.

    The trash receptacles in both the men and women's restroom are full. The women's trash receptacle is overflowing.
    Garbage and rubbish shall be disposed daily or at a frequency which does not create nuisance.

This is a re inspection due to excessive flies seen in facility. At the time of re inspection, minimal flies were observed. Be advised that since no flies were seen during this re inspection, based on the recent history of fly activity, this does not mean that the flies are gone for good. Routine cleaning must be continued and proper maintenance should be performed. The floor tile adjacent to dump sink was still loose with water sitting underneath it. This must be addressed. Any sitting food/drink spills, leaks, spalshes must be cleaned up to deter pest harborage. Any sitting water must be cleaned up. Ensure that the drain line for the ice bin is lined up with floor drain so water does not sit on floor for long periods of time. It is still recommended that the cabinet floor under the dump sink be cut out to allow for greater access when cleaning. Furthermore, if deemed necessary by this Department, if this violation is seen routinely, a comliance conference may be requested.

Pest control report from American Pest Control was provided from 6/11/12.
  • Critical: Presence of insects/rodents. Animals prohibited
    very few flies were seen at mop sink area. The previous violation is corrected. Continue with routine and rigorous cleaning schedules.
6/11/20121st Recheck
HACCP: Discussed keeping back area hand sink clear to wash hands.

The fly problem was excessive during inspection. Flies were observed heavily throughout facility. It is recommended that the ice bin be pulled out routinely to clean out area. Every nook and cranny that you can think of needs to be clean. Schedule extra cleaning shifts if necessary to clean and sanitize areas. Regular cleaning schedules will not suffice. Any water or food source must be cleaned or repaired and maintained. By taking away the food and water source, the flies will not thrive. Clean any area that can be a splash area, such as walls and in between equipment. Lips of countertops and any other area that could potentially be a splash area must be cleaned. Fill the mop buckets up only when in use. Dump water out when not in use. Contact pest control operator ASAP to service facility. Reinspection will be in 7 days. If excessive numbers are still seen, the owner will be mandated to attend a compliance conference with the Asst. Director and Director of Environmental Health.

    Non-food contact surfaces of equipment shall be cleaned as often as is necessary to keep the equipment free of accumulation of dust, dirt, food particles, and other debris.

    Observed water coming from underneath ice bin.
    Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.
    Ensure that drain lines go directly to floor drains.

    Obseved excessive fly population throughout facility, including the prep area, mop area, throughout dry storage area, walls, and ceilings.
    Effective measures to minimize the presence of rodents and flies, roaches, and other insects on the premises shall be utilized.
    Reinspection will be on or after 6/11/12.

This is a third reinspection due to excessive fly population. At time of reinspection, minimal flies were seen. Continue routine cleaning and scrubing floor drains.

    Observed minimal flies in establishment. Continue routine and rigorous cleaning in and around dump sink area and floor drain, especially hard to see/reach areas. Violation corrected.
4/3/20123rd Recheck
Re inspection due to excessive fly population, especially under dump sink. Heavy numbers are still seen under dump sink, on walls, and around blender area. According to employee, facility was serviced last week by pest control operator. CB-80 insecticide is being used to spray drains and non food contact surfaces. Employee stated that pest control operator recommended that the lower part of the cabinet in front of the dump sink drain be cut away for greater access to the dump sink drain. This area seems to be where the highest activity is noted.

Re inspection fees will be assessed to each additional re insepction.

    Excessive numbers of drain flies observed under dump sink and on walls at blender area. Employee stated that pest control serviced facility last week. CB-80 insecticide is being used to spray non food contact surfaces and drain areas. Fly strips are being used under cabinets.
    Effective measures to minimize the presence of rodents and flies, roaches, and other insects on the premises shall be utilized. Re inspection on or after 4/3/12.
3/20/20122nd Recheck
This is a re inspection to check for flies. Flies are still observed. Work with pest control operator to resolve problem. A pest control report is expected to be provided for next re inspection.
Fly strips will help control the adult population but will do nothing for breeding. Use pest control to target breeding spots. Re inspection fees will be assessed.

    Flies are still observed throughout facility-underneath dump sink, three compartment sink area, and mop sink area.
    Effective measures to minimize the presence of rodents and flies, roaches, and other insects on the premises shall be utilized.
    The fly strip in the back storage area needs to be removed. No fly strips over or area any food or food storage. Re inspection will be on or after 3/19/12.

    Observed cleaner stored intermingled with customer single use items on storage shelf nex to freezer.
    Poisonous or toxic materials necessary for the maintenance of the establishment shall not be stored above or intermingled with food, food equipment, utensils, or single-service articles.
    Store all toxic items segregated from food or food items. Violation corrected by moving cleaner.

This is a second recheck from a previous violation for excessive fruit flies in facility. Minimal flies were observed. A pest control report from 12/23/11 from American Pest Control was provided at time of re inspection.

Informed employee to continue with sanitary practices throughout facility and if deemed necessary, could put fly strips under three comp sink or under dump or hand sink cabinets in service area to help control adult population, granted the strips are not in an area that may contaminate food or other food contact items.

Continue monitoring for pest activity and inform your pest control operator of areas of concern in order for him to treat effectively.

    The previous violation of excessive fruit flies seen throughout facility was corrected. Minimal flies observed. Continue with routine and rigorous employee sanitation in facility, not limited to scrubbing floor drains and improving cleaning practices. Continue to monitor fly problem and inform pest control operator of problem areas.

This is a re inspection from a routine conducted on 12/2/11. Fly activity was still notice at the areas mentioned on the following page. Provide pest control report for next reinspection. Increase measures to minimize pest activity.
