No evidence of mice or other vermin. Mouse traps that were visible were empty. * Recommend hanging hose in mop sink on a hook on wall so it does not terminate inside mop sink.

    320) Garbage enclosure area is dirty. Clean thoroughly to prevent vermin harboarge.Clean all loose garbage and place all garbage bags inside garbage dumpster/container.

    Observed loose trash and broken equipment around perimeter of store adjacent to garbage dumpster. Facility shall pick up all loose trash; broken glass; paper and any equipment that is not in good repair and not used in the operations of the business into the garbage dumpster. Do not allow broken equipment to collect outside. This will prevent vermin attraction and harborage.

    Observed employee breakroom dirty. Facility shall keep employee break room clean and sanitary. Do not allow food debris to collect on tables and countertops. A dirty employee break room is a source of cross contamination and a breeding ground for microorganisms that can cause illness.Do clean and sanitize breakroom; diaper changing pad and other high touch surfaces with chlorine and water. Use 1 teaspon of water to 1 pint of water or 1 cap of bleach to one gallon of water.

No chemicals next to food.Do rotate foods first in first out method of rotation and monitor use by dates/sell by dates for "best practice." Washoe County has no regulations regarding use by dates.Bathrooms observed clean and sanitary.

    Observed men's bathroom with heavy scent of sewer gases. Discussed with operator to pour water into floor drain located in hallway outside of men's bathroom and disinfect floors in men's bathroom with bleach and water. If odor still exists after one week; facility is to investigate if odor is a result of urine rotting subfloor underneath old floor tiles and replace floor in men's restroom within 90 days.
