General Comments that relate to this Inspection The items noted on the previous inspection conducted on 5/28/2015 have been corrected.Ok to issue permit to operate.Provide the following information on vending machine and inside of facility. Business name; phone number; water obtained from approved public water source; type of containers that are approved for use in the facility.Water from point of dispensing will be tested every six months for total coliform and total disolved solids. Keep water sample reports filed on site for last two years.Plans to open on 6/6/2015.

General Comments that relate to this Inspection The following items must be completed by the next inspcetion for issuance of health permit.1. Provide soap and paper towel dispensers for hand sink and stock with soap and paper towels. 2. Provide separate drain lines for hand sink; mop sink and 3-compartment sink; each sink needs to have individual drain lines. 3. Install splash guards in between all in-line sinks. 4. Install a RP backflow preventer upstream of filtration system. 4. Finish installation of bottle filler table and vending machine. 5. Caulk holes on walls at pipe penetrations. 6. Provide soap and sanitizer and test strips to be used for 3-compartment sink.Carpeting in back room must be removed to provide washable; smooth and non-absorbent floor surface by 6/30/2015.
