    -Observed chicken wings and dough in walk-in refrigerator uncovered. Except during cooling; all food products must be covered to prevent potential contamination.-Observed no date marking of prepped food products. All items must be labeled with the discard date that may be no more than 7 days after opening/preparation; unless the food product will be used within 24 hours.-Observed employee touching cooked pizza with bare hands during cutting. Employees shall not touch ready-to-eat foods with bare hands.- Observed raw chicken being stored in a merchandiser fridge that is specifially labeled to be used for prepackaged or bottled products only. This unit shall be replaced with an NSF approved unit for potentially hazardous foods or the food shall be relocated to a unit approved for storage of potentially hazardous foods by December 31; 2016.

    Observed various areas of non food contact surfaces dirty. All non food contact surfaces of equipment must be clean. These areas observed include: floors below ice bin and on floor below soda machine and along syrup lines.

All non food contact surfaces of equipment must be clean.-Observed the area behind the dough mixer with a build up of food product. -Observed the seals for the make up unit with a build up of food products. -Observed a build up of food product and and dirt behind equipment in the kitchen and storage areas.The following violations are non-food contact maintenance items that would normally be marked under a separate violation code. Due to technical issues these items are being combined with the non-food contact cleaning violation code: -Observed the shelves in the dry storage area with exposed wood. These shelves shall be repainted or sealed to ensure they are smooth and easily cleanable. -Observed velco on the dry good storage bin edges. Velcro needs to be removed so that the surfaces are smooth and easily cleanable.-Observed bungee cords being used in the stand up reach in freezer. These cords must be removed and replaced with something that is smooth and easily cleanable.

    Observed wet towels laying on various surfaces throughout kitchen. Moist wiping cloths must be kept in sanitizing solution with 50-100 ppm Chlorine or 200-400 ppm Quaternary Ammonia between uses to prevent bacteria growth.

    The facility also has the following Certified Food Protection Managers (CFPM): Natalie Koehler-Servsafe 9885335 exp: 03/25/2018; Jeffery Peterson-Servsafe 11684582 exp: 10/29/2019; Amy Brant-M150004 exp: 12/17/2019.One employee has taken the CFPM exam within the past year and does not know if they have passed.Discussed no bare hand contact with ready-to-eat foods.Discussed employee health and that staff with gastrointestinal illness (vomiting/diarrhea) may not work for 48 hours after symptoms have stopped.No pest issues noted during inspection. Ice machine okay; ice scoop properly stored.Hand sink stocked.Discussed proper set-up of 3-compartment sink with staff. Test strips available. Discussed proper sanitization of all food prep areas.Observed good employee handwashing.Chemicals stored properly and labeled.Trash okay.Observed the following temperatures:-Salad bar: cheese 41F; mushrooms 37F.-Walk in refrigerator: chicken wings 37F; cheese 38F; onions 37F; sauce 38F.-Reach in refrigerator (on prep line): 38F; sausage 37F; pepperoni 41F; garlic spread 41F.-Pizza make up unit: sausage 39F; cheese 39F-Deep chest freezer: 0F.-True reach in refrigerator: 39F; sliced potatoes 39F; chicken 38F & 40F.-Stand up reach in freezer: -10F.

    Hand sink clean and stocked. Restroom clean and stocked. Reach in @ 28F. Dairy reach in @ 39F. Good ice scoop storage. Discussed three comp sink set up with operator. Chlorine utilized but sink not set up during inspection. Test strips available. Chemical storage OK.*Observed ice build up in very bottom of glass reach in unit. No contamination observed but recommend getting unit serviced.
