160) Had two dishwashers on the line one was completely broken and had not been working wither repair/replace or remove. The one that was supposed to be working (2 hours into opening) would not start when opened to look there was no water in it. I filled it and started it could not get sanitizer reading checked sanitizer and bottle was empty. Glass in it. Granted a majority of service is plastice cups.

210) Both handsinks were being used as dumpsinks; need at least one designated as a handsink. Problem was both sinks were over half full of drink debris and ice.

220) The main refer on the line had mold and algae build up along with multiple spilled items in the bottom.Handles on the refer and face of most equipment were all sticky; should be clean to pref=vent cross contamination.

240) Bartender on far right side had 5 plastic cups in the ice as scoops. Claimed didn't know couldn't use cups. Must use scoops with handle and handle kept out to prevent cross contamination of customer ice. (Bartender was grabbing money; and never washing hands while useing these cups)

Notes:Manager was a pulled in manager who was mainly there to focus on the club and the FB Manager was a week new in position. Facility was very poor; very busy but still needs to be maintained bar tenders over whelmed no bar backs. Floors filthy; equipment filthy; handsinks full of drink debris; dishwshers not working; no hand washing; improper scoop use. Went over all the concerns and need to re assess the way is ran.

Repair dish machine and faucet before New Year's Eve Party. Found two ketsup bottles behind bar. While food service is allowed at private parties; no food can be served when facility is open to the pubic as this is a smoking bar. (Ash trays noted. No smoking sign have not been posted.)
