**This permit is not in use currently. It could apply to one cart that is used only about 2 x per year or it could have originally been used for the cart that has been outsourced to Arroyo. Levy Restaurants has numerous extra Certified Food Managers to operate the cart should it be necessary. Permit may be closed as all Levy permits are being re-evaluated to be current.

Food served is hotdogs; hamburgers; baked beans and potato salad. Reviewed employee sick policy with operator. Operator knows when to exclude ill employees and when employees may return to work. Operator request word addition to health permit to read "catering."Do post health permit on unit for identification.
