**This bar is no longer used and is completely shut down. Operator wishes to leave the permit in place just in case they decide to open it up for New Years. **Instructed operator that the following must be completed prior to any opening:1) Complete cleaning and sanitizing of bar area2) Ensure that all hand sinks are stocked3) Ensure that dishwashing is being done with proper sanitizer concentration

**Facility is not in operation and is not used even during busy times. Operator does not wish to close out permit just in case there is a time that he does wish to reopen prior to planned renovation of property.Several issues were observed with the condition of the facility that should be corrected even if no operations are to be conducted:1) Mobile ice bin was left filled and was observed to have a lot of mold build up. Bin should be cleaned and sanitized and left dry to prevent these issues.2) Speed guns were not cleaned on the closing and had a lot of build up on them. Clean guns thoroughly.Finally; prior to reopening for regular operations; the flooring issues should be addressed. The floor is pitted concrete which really would not allow for proper cleaning. Operator will discuss with Inspector should the facility be reopened prior to complete renovation.

**This facility has been closed for approximately 6 months. On very busy days; like major holidays; it is opened as needed. Reviewed the following opening procedures with operator for those times when it will be used: Checking dishwasher sanitizer concentration; thorough clean and sanitizing of surfaces and glassware; cleaning speed guns.
