    Observed ledge on ice machine and inside of ice machine dirty. Do clean and sanitize ice machine of all bi-carbonate buildup and mold growth on a regular basis. Ice shall be burned off or removed and entire interior washed down and sanitized and flushed with clear clean water at least quarterly.

    170) Food contact surfaces dirty. All food contact surfaces must be clean and free of debris.Observed bar gun and holder dirty with buildup. Do clean bar gun and holder on a regular and routine basis to prevent mold growth.

All 5 point violations listed on this inspection report are critical items and have to be corrected if deducted in order for facility to operate.

    Please acquire Quaternary ammonia test strips to test Quat sanitizer used at 3 compartment sink. This violation continues to exist. Do test quat sanitizer used at 3 compartment sink daily to monitor for proper levels of Quat sanitization to be at levels between 200-400 ppm.Chlorine or bleach may also be used to sanitize glassware and needs to be monitored to be at levels between 50-100 ppm.Test strips can be acquired at Resco and Cash n Carry or any chemical supply store/vendor.

    Acquire a sanitizer bucket to store wet wiping cloths in a sanitizing solution of either Quat at 200-400 ppm or Chlorine bleach at levels between 50-100 ppm. This violation continues to exist.

    All other violations have been corrected at time of reinspection. Please call health inspector on or before 12/10/14 when all items noted have been corrected at 328-2682. Failure to correct violations by abatement date noted on inspection report will result in reinspection fee being assessed and possible suspension of health permit.

    Observed deflector plate inside ice machine dirty. Ice machine shall be cleaned on a regular basis. Ice machine should be cleaned and sanitized on a quarterly basis. Ice shall be removed and inside unit properly cleaned and sanitized including filters; working parts; deflector plate; etc.

    Observed bar gun holder dirty with visible mold growth. Clean both bar gun holder and spigot on a daily basis to prevent mold growth.

    Axquire Quaternary ammonia test strips used at 3-compartment sink. Quat sanitizer shall be monitored to be at levels between 200-400 ppm. This should be checked daily using test strips.

    Observed broken glass at bottom of cold holding beer bottle bunker. Remove all beer and remove all broken glass from unit.

    All violations corrected at time of reinspection. This bar is located in a strip mall and a mop sink is available approximately 25 feet from the actual bar's physical location. The designated mop sink for this bar is located in strip mall hallway. All employees must use this mop sink room for any mopping operations. Reviewed with operator that any spills or PVI public vomiting incidents that require a wet mop need to be done using the designated mop sink. No water may be dumped down the handsink in the bar or in any plumbing fixture in either men or women's bathroom. Please post a handwashing only sign at designated handsink next to ice machine. Handwashing may not occur at 3 compartment sink. Handsink is a utility style sink located next to ice machine and in a separate room from main bar area. This needs to be done by 08/08/13.

    Observed handsink in back room next to ice machine blocked with beer kegs. All hand washing sinks must remain unobstructed and accessible during operation to ensure proper hand washing. * Corrected on-site.The handsink in backroom next to ice machine is also used as the mop sink. A handsink shall be installed inside the bar area per regulation. Section 090.110 of Washoe County regulations states: Sinks used for food preparation or for washing equipment or utensils must not be used for hand washing. The 3 compartment sink cannot be used as a handsink. Section 090.105 states that an approved number of lavatories/hand sinks must be provided and must be installed according to the current edition of the Uniform Plumbing Code and must be located to permit easy access and convenient use by all food/beverage handlers within food preparation areas and utensil washing areas. A mop sink cannot be used as a handsink. Mop water cannot be dumped down handsink or toilet. A designated and separate hand sink and mop sink shall be in place at facility. Handsink must be located within 25 visible feet of any food/beverage service areaPlease contact heatlh inspector at 328-2682 to discuss options for installation of required handsink. This bar has operated without a designated handsink in main bar for many years.
