    Noted hand sink stocked and functional.Pretzel hot dog at 137 F in warmer case.Quat sanitizer checked at 200 ppm concentration in 3-compartment sink.Quat test strips kept on site for sanitizer monitoring for proper concentrations.Noted floor and equipment to be kept clean in general; just ensure detailed cleaning along floor and wall junctures under ice maker and refrigerator to rid of debris build-up.No signs of pest noted.No glue traps provided for monitoring of vermin. Have pest control company set traps for proper monitoring.There is one CFPM employed in this facility; NRE of FSP certificate issued 1/28/2016; CFPM obtained WCHD certificate on previous certifcation. Facility no longer required to have a CFPM due to risk category 1 assigned.

    The items noted on the previous routine inspection condcuted on 2/27/2015 have been corrected.Glue traps noted with roaches have been removed. One glue trap that was not noted with few dead roaches on the last inspection found inside cabinet where water heater located. Remove and replace glue trap. Ensure to contact pest control company for service as needed to keep roaches out of establishment.

    Two glue traps with cockroaches on them noted insde soda cabinet and 3-compartment sink cabinet. Contact pest control company for service. Replace glue traps when loaded.

Wipe and clean the cabinet shelf under 3-compartment sink where water is observed. Ensure that water leak has been fixed.

Floor under ice machine and refrigertaors may need cleaning more frequently to minimize build-up of food denris; at this time; light amount of crumbs observed.Dirt and drink spills noted in bottom surfaces of cabinet under drink dispensers; dirt and debris build up noted inside enclosue for cleaning tools storage. Add these areas to cleaning schedule to have them cleaned with a set frequency.

    Refrigerator at 35 F.Pretzel with peppperoni at 179 F on hot display case.Cleaning of floors and under equioment and inside cabinets is much better than on previous inspection. Ensuring to keep all the areas clean and routine pest control service has abated vermin activity.Noted hand sink stocked and functional.

    The items noted on the previous routine inspection and follow-up inspections have been corrected.Facility received treatment for cockroaches from pest control operator last Wednesday.Continue cleaning of floors and cabinet surfaces periodically to keep them free of any food that may attract cockroaches and have pest control operator treat the facilty as often as needed.
