Observation: Food Inspection Report not posted at time of routine inspection.
- Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
Observation: No verifiable Employee Health Reporting Agreement available onsite. Person in charge printed form 1B and had staff read and sign reporting agreement.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation:No thermometer present inside the two door pizza prep cooler.

- PHF/TCS food that is cooked and cooled on premises is reheated within 2 hours to 165°F or above for 15 seconds for hot holding
Observation: Pre-cooked chicken strips and hamburgers were not reheated correctly prior to placing into the hot holding case. Chicken strips (91 degrees) Hamburgers patty (101 degrees. PIC changed setting on the oven and reheated the products to 165 degrees.

Observation: Mop heads drying on hand sink splash guards at time of inspection. Corrected by PIC.
- Sanitization of food contact surfaces - before use and after cleaning
Observation: No quat sanitizer solution mixed up at time of inspection in the kitchen while food preparation was taking place. Corrected by PIC.

Observed hand washing sink next to mop sink was blocked by the mop bucket and mop. Observed interior of the hand washing sink was soiled with food residue. Observed food handler rinse out a cup at the hand sink next to the 3-compartment sink.

Observed single use items such as clamshells and single-use cups stored on the ground in the kitchen. Observed container of sausages stored on the ground of the walk in freezer. Observed milk stored on the ground of the front reach-in refrigeration units.
