Observation: All hand sinks used by food service employees must have a hand washing sign posted.

Observation: Clean the top of the "Sunday Topping" cooler.

Observation: Black plastic rack and some wire racks in kitchen are not 6 inches off the floor. Must raise all racks to have lowest shelf 6 inches off the floor.
- Material characteristics of non-food contact surfaces
Observation: Packing tape on ice machine has not been removed. Must remove. Also the wood cabinets in kitchen (by front service counter and mop sink) have bare wood exposed and must be sealed or painted.

Observation: True freezer by the back door has a burned out bulb.
- Drying mops
Observation: Do not have a mop rack installed.
- Plumbing system maintained in good repair
Observation: The collar sink drain line by the drive-thru window must be adjusted to prevent splashing on the wall and floor.
- Equipment location, installation, repair, and adjustment
Observation: Door gaskets on the Norlake cooler (Sunday Topping) are torn. Must replace both gaskets.

Observation: The drip tray at the front counter (collar sink) must be abandoned properly.
- Cleanability of floors, walls, and ceilings
Observation: The following areas are not smooth, easily cleanable, and non absorbent: all floor drains, cove base @ front counter shelving supports, underside of front counter top, underside of half wall @ front counter, all kitchen floor drains, stainless steel shelves by drive thru window, supply lines to ice cream machines, and mop sink. All areas must be sealed.

Observation: Ceiling above Dilly Bar chest cooler is damaged. Must repair to maintain it in good condition and prevent entry pest. REPEAT VIOLATION

Observation: Can foods stored on the floor at outside of walk-in cooler. Food must be 6" off the floor to prevent contamination and cleaning purposes.
- In-use utensils, between-use storage
Observation: Cup being used as scoop in condiment container (Butterfinger, and M&M's) on the shelving in back storage area. Must use utensil with handle stored outside of food and keep inverted to prevent contamination.

QA test stripes was expired. Order a new test kit to ensure sanitizer concentration maintained between 200 - 400 ppm.

Paper towel and several boxes of single service items were stored on the floor in unused walk in cooler. Single service and use items must be stored 6" off the floor.
