- 3.2 White substance in two containers were unlabled which looked like pretzel salt and parmesan cheese/oregano mixture. Label containers to prevent confusion/misuse.
- 4.3 Area between wall and counter behind coffee maker needs to be re-caulked. Please make necessary repairs to provide a smooth, cleanable surface.
- 4.1 No thermometer in pepsi cooler. Provide thermometer to monitor proper temperatures of 41°F or below.
- 6.2 Light out over ice machine in storage area. Replace bulb to provide proper lighting levels.
- 6.0 Counter damaged in men's restroom by the lockers. Make necessary repairs to provide a smooth, cleanable surface.

- *2.4/4.1 Temperature measuring device was reading 35°F. Temperature measuring devices shall be accurate within +/-2°F to monitor the temperatures of potentially hazardous foods.
- *7.1 Observed Chafing fuel was stored with equipment. Toxic chemicals shall be stored in a separate designated area from equipment to prevent contamination.
- 4.5/6.4 The following areas were soiled:
- - floor under the pop syrup rack
- - continue to clean the floor surface under the bottled beer in the walk-in/reach-in beer unit
- - continue to clean the floor behind the bar ice bin
- - ceiling vents and the surrounding areas are dusty
- Clean these areas.
- 6.4 Observed employee hat was stored on the equipment rack. Store employee personal items in a separate designated area.
- 4.8 Store the pizza boxes six inches off the floor to facilitate cleaning and to prevent contamination.
- 6.4 Repair the unsecured floor in the beer walk-in cooler, to provide a cleanable surface.
- 4.4 Repair the damaged door seal on the upright Beverage Air upright unit, and the Gibson upright freezer.
- 4.4 Repair the Gibson upright freezer which contains an air temperature of 37°F, and the foods were holding at 32°F. Food shall be maintained frozen and air temperature of freezer units shall be maintained at 32°F and below.
- 4.4 Repair the damaged strip on the top interior left side of the Beverage reach-in cooler.

- *3.4/4.4 The beer walk-in cooler contained an air temperature of 43°F and the creamer in this unit was holding at 44°F. Food and refrigeration units shall be maintained at 41°F and below to limit the growth of organisms.
- 4.5/6.4 The following areas were soiled:
- - the floor surface under the bottled beer in the walk-in/reach-in beer unit - floor behind the bar ice bin
- - ice accumulation in the pizza reach-in unit - two sleeves from the thermometers
- - white pipe draining into the preparation unit floor drain - Clean these areas.
- 4.8 Store the pizza boxes six inches off the floor to facilitate cleaning and to prevent contamination.
- 6.4 Repair the unsecured floor in the beer walk-in cooler, to provide a cleanable surface.
- Note:
- Operation is not using the Gibson upright freezer unit.
- Observed coleslaw contained a sell-by, use-by date of 10/3/10. Deli salads shall be used or discarded by the manufacturers date.
- This product was discarded today.
