5) Quat test strips provided

Final approval is given to operate under Chapter 10, Food Safety, of the Peoria County Code.  License #15-1121 was returned to establishment."

On-site inspection conducted in response to complaint regarding unapproved construction/remodeling of establishment during operation.  Upon arrival for inspection, the following was noted:

1) No hot water is available in the establishment

2) No restroom facilities are available in the establishment

3) Three compartment sink is not properly plumbed and is draining into a bucket

4) Entire dry storage and prep area has been demolished and dust is being tracked throughout establishment

5) Plans for remodeling have been requested multiple times (through email, phone calls, and on inspection forms) and have not been received or approved

License #15-1121 has been suspended for this establishment until approval is given to reopen by the Health Department.  A Plan Review and Plan Review fee of $200 must be received as soon as possible by this Department to proceed with construction/remodeling.

Facility is planing on doing a remodel of facility while remaining open. Spoke to Chris (district manager) about submitting floor plans and plan of action to assure no food will be contaminated during remodel process  to PCCHD as soon as possible.

Facility must have at least one certified food manager at this establishment. All food handlers must have an ANSI approved Food Handler Certificate.
  • Food contact surfaces designed, constructed, maintained, installed, located.
    Found duct tape on plastic food container. Manager discarded container. Several of the plastic squeeze bottles are stained.
    Food-contact surfaces shall be smooth and easily cleanable, free of breaks, open seams, cracks, chips, pits, and similar imperfections, and free of difficult-to-clean internal corners and crevices.
  • Non-food contact surfaces designed, constructed, maintained, installed, located.
    Please paint the wooden shelf by the computer area. Please paint the woden blocks under PVC drain pipe to three comp sink. Shelf above three comp sink and some of the shelves in the make table have metal exposed and metal is starting to rust. Non food contact surfaces shall be easily cleanable and in good repair.
  • Plumbing installed and maintained
    It does not appear that facility has a grease trap. A referal will be sent to the plumbing inspector.
    Grease traps shall be located so that the trap and its surroundings are accessible for cleaning.

    Found missing tiles on floor by the front door. Found missing cove base by the hand sink in the back kitchen area. Floors shall be in good repair.

    Clean the back side of the hood where the exhaust comes out of the black pipe for the lower oven.
    Intake and exhaust air ducts shall be maintained to prevent the entrance of dust, dirt and other contaminating materials.
