    CFPM has not been completed. Has not met the 60 days compliance date for completing serve safe course. Will be given 30 more days to complete if not completed in given time frame Health Permit to Operate will be temporary suspended until completed. Shannon has sign up for serve safe class on 8/25 & 8/26. Once completed must bring serve safe certificate to WC Health District and receive WC food managers card. Complete by 9/8/14.

    The following items must be corrected prior to permit issuance:-remove duct tape from keg cooler and replace gasket-seal/paint exposed wood shelf under glass storage area-seal the space between the dump sink and the dirty glassware drain board. Ensure surface is smooth and easily cleanable-Ensure all non-movable equipment (dump sink; three-comp. sink) is sealed to wall.-re-caulk around sink in unisex restroom and men's restroom-ensure all cold holding units are equipped with functioning thermometers-clean facility including inside reach-ins and behind/under equipment
