All non food contact surfaces of equipment must be clean.Observed stereo in prep area dirty with food stains and in need of cleaning to prevent cross contamination issues. Discussed with manager to have it either cleaned on regular basis or to remove to prevent any issues. Correct by the end of 3/10/16 or when operation slows.
[1] Storage; handling of clean equipment / utensils 230) Improper storage of utensils.Observed used scoops hanging in rice prep area. Scoops had lime juice; used cilantro caked on scoops - liquid dripping on tables above clean food containers. Discussed with managers about switching scoops out to clean scoops everytime when scoops have been used. Managers corrected issue on site.
[1] Floors; constructed; drained; clean; good repair; covering installation; dustless cleaning methods 350) Floors dirty. Clean thoroughly to prevent vermin attraction and maintain sanitary conditions.Observed in walk in cooler puddle of red liquid below shelving. All floors must be clean and sanitized on regular basis. Clean by the end of 3/10/16 or when operation slows down.

Discussed no bare hand contact to ready to eat food items - no issues observed - observed frequent glove changing. Observed frequent handwashing onsite between duties.
