Observed missing thermometer at reach in beer bottle cooler. Butter and cocktail sauce is being stored in same unit and must be monitored for proper cold holding at 40 F or lower. Recorded temp. of cooler by health inspector observed at 39 F with thermocoupler.

This bar is an age restricted bar. Only 21 years or older persons is allowed in facility. This is a smoking bar. Shrimp cocktails; shrimp scampi and "tombstone" pizzas are served. Both food items are cooked on premise. Shrimp is boiled and pizzas are being heated in toaster oven in back room of bar. Utensils and cooking equipment must go through 3 step cleaning process: wash first with soapy water than rinse with clear water and than sanitize with Quat sanitizer monitored to be at levels between 200-400 ppm.Quat sanitizer recorded at 200 ppm at 3 compartment sink.All cold holding checked OK. All chemicals properlys stored and labeled.Handsinks properly stocked with soap and sanitary disposable towels.

All food properly stored. All handsinks properly stocked with liquid soap and sanitary disposable towels.This is a smoking bar. Do post the alcohol warning in women's restroom. Printed a copy for operator to post.Discussed with operator to remove shrimp cocktail sign advertised outside of bar. Reviewed Nevada clean indoor act requirements with operator. No food is to be served to public in a smoking bar. Bar must be closed to the general public if a private party with food is involved in a smoking bar. This is a requirement for operating you Snackbar health permit.
