Observed burritos in reach in fridge with expiration dates of May and June. Items discarded during inspection. Ensure expiration dates are monitored and product is rotated appropriately. This item considered corrected on site but will be reinspected when other items are checked.

Observed bagged ice not labeled with name and address of facility. All bags must be labeled with name and address of facility where ice is produced.

Observed items marked "Keep Frozen" in reach in fridge that were completely defrosted without a date marking system. All items discarded during inspection. These items are not to be allowed to defrost fully. This item considered corrected on site but will be reinspected.

Observed shelves under soda machine dirty with dust and debris and old mouse droppings that were possibly not cleaned up from previous owners. Ensure these shelves are cleaned with a bleach water solution and then monitored to ensure no futher mouse problems exist.

Dairy fridge @ 41F. *Replace burned out light bulb in this unit. Good product dates on dry goods grocery items. Ice machine clean. Good ice scoop storage. *Repair slow leak at three compartment sink.

Items noted on previous inspection have been corrected with the exception of the following: - Repair broken door frames to 3-comp sink room and bathroom. - Repair broekn floor sections in 3-comp sink room and sand; seal and paint entire floor in this room to ensure a smooth and easily cleanable surface. - Repair broken sections of linoleum this throughout facility to ensure a smooth and easily cleanable surface. The following is a new item: remove old caulking around three comp sink and re-caulk sink to wall. The above items must be corrected within 90 days by 7/15/15. Ok to issue permit to operate. Facility will be Risk Category 1- no CFPM required.

New owner is waiting for liquor license approval to take over business. Once this approval is received owner shall contact WCHD inspector. At that time the new owner shall ensure the following itmes are corrected within 14 days unless otherwise noted: - Cover electrical outlet at entrance to walk-in. - Repair broken wall flashing inside walk-in. - Deep clean all shelves inside walk-in. - Repalce burned out light bulbs inside walk-in. - Deep clean walk-in floor. - Install light bulbs in Coke and 7-Up fridges. - Remvoe or replace wooden shelves next to 7-up fridge. Must be smooth & easily cleanable. - Repair corner edging where missing near entrance to walk-in. - Repair broken and missing coving around walk-in exterior. - Replace gasket to door on far rigth of walk-in. - Remove carpet remnant from in front of soda machine. - Deep clean all shelves; cabinets under coffee/soda machine. - Replace toilet seat in restroom. - Deep clean three comp sink and hand sink and remove all items not used in daily business. - Repair broken door frames to 3 comp sink room & bathroom. (90 days)- Deep clean floor drain in liquor storage room. - Remove or replace broken lid to free standing beer cooler. - Repair broken floor sections in 3 comp sink room and sand; seal and paint entire floor in this room so it's smooth and easily cleanable. (90 days)- Repair broken sections of linoleum tiles throughout facility so floors is smooth & easily cleanable. (90 days).
