Noted hand sinks stocked and functional in all areas and restrooms except for the hand sink draining slow.Refrigetaion units checked at 40 F or below. Quat sanitizer checked at 400 ppm in 3-compartment sink and 200 ppm in sanitizer bucket and pans. Sanitizer kept on timer and changed every hour.Dish machine temeprature observed at 181 F on sanitization cylce.Ice machine interior noted in clean condition; ice machine is cleaned and sanitized periodically.Noted common trash enclosure of strip mall clean.

All refrigerators noted at below 40 F and freezers at 0 F.Dish washer at 140 F on wash and 183 F on final rinse cycle.Noted hand sinks in prep areas and in restrooms stocked and functional.Quats sanitizer checked at 200 ppm concentration in buckets and pans with wiping cloths. Sanitizer changed out every hour.Floor areas under equipment noted much cleaner than on last year's routine inspection. Continue doing frequent scheduled wet cleaning of floor there to keep them clean.

There was a general accumulation of food debris on the floors (behind and around trash cans; under equipment agains the walls in hard to reach ares; counter tops; etc.). Clean and maintain clean.

OBSERVATIONS; RECOMMENDATIONS & REQUIREMENTS:- All sanitizer levels observed were at or near regulation (200ppm quat at wipe down bucket water); 185F rinse water temperature on dish machine. Ensure sanitizer wipe down water is changed frequently enough to maintain sanitizer levels and when visibly dirty. Water was visibly dirty at the time of inspection however the water was disposed of and remade during the inspection.- All refrigeration was at or near that required by regulation (large 3-door reach-in cooler 34F; reach-in freezer -10F; display cooler 39F).- All hand sinks were stocked and operational.- Observed good employee hygiene (handwashing; personal item segregation) at the time of inspection.

No violations observed at the time of inspection.OBSERVATIONS AND RECOMMENDATIONS:- All sanitizing equipment within regulation. Dish machine 172F at the dish; sanitizer wipe down bucket 150ppm quat.- All ambient temperatures were at or near regulation.- All chemicals were labeled. Ensure that once you use the chemical you return it to the chemical storage area rather than storing it on the counter top (e.g. sanitizer on the counter area).- Reviewed previous PCO treatment log. Service performed by Integrated Pest Management on 5/2/11. No activity noted.

NOTES: - Sanitizer concentration = 200ppm quat in sanitizer buckets. Ensure that sanitizing solution is made up prior to shift start and then change out every couple of hours or more often if visibally dirty. - All ambient refrigeration 40F or less with thermometers installed. - Dish machine rinse temperature at the jacket = 183F - PCO preventative maintenance monthly by Integrated Pest Management. Reviewed last treatment invoice on 4/6/10 - noted light gnat activity on light trap. No activity on glue boards.No Violations Observed at the time of inspection.

No violations observed at the time of inspection.NOTES:Most recent pest inspection report on file. Checked 2/9/09 by Western Extermination. No notes on PCO inspection report. Store checked as clean. All hand washing sinks were fully stocked. All ambient refrigeration temperatures were within limits with no expired product. Store was clean and well organized. Please make sure all containers of white powders are labeled to prevent misuse.
