    This is a repeat violation from initial inspection on 01/24/08. Observed calcium carbonate build-up along the deflector shield (bottom half of ice machine). Put ice-machine on a monthly cleaning schedule to prevent physical contaminants coming in contact with ice.

    Observed dish sink and surrounding plumbing sink area leaking. Facility is in process of having sink replaced. Have source of leak investigated; repaired as necessary to prevent leaking water. Facility to call inspector when all repairs have been completed for sink leak. 775-328-2682.

    Clean mold build-up inside trays (in upper half od machine). Also; clean calcium carbonate build-up along the deflector shield (bottom half of the machine) and upper half of machine. Put ice machine on a monthly cleaning schedule.

    Provide Quat test strips to measure sanitizer.

    Found no measurable Quat sanitizer in the cloth bucket. Sanitizer was changed-out and measured OK at 200 PPM. (corrected on site)

    Inspector and staff observed water dripping from behind the "yellow"-colored panel behind the plumbing under the dish sink. Locate the source of the water and repair the leak and keep this cabinet area dry and clean. Clean floor around the ice machine; also.
