Observation:Is not wearing a hair restraint during food prep, must wear a hat, visor, or net while handling foods.

Observation:Ice scoop handle is touching ice, must keep handle inverted out of ice, corrected.
- Test kit provided and used to measure sanitizing solution concentration
Observation:Does not have any test strips for quat sanitizer, does have for chlorine, used in three compartment sink.

Observation:Does not have reminder statement on menu, must be on one page. Both of these may be written or use of sticker until ready to reprint menu.
- Separation from food, equipment, utensils, linens, and single service
Observation:Has spray bottles of cleaner hanging on side of clean end of warewashing sink, must move to lower area where other chemicals are kept separate from foods.
- Bare hand contact with ready to eat foods
Observation:Had run out of gloves, now restocked, must keep adequate supply in reserve to ensure has available for usage.
- Equipment and utensils are adequately rinsed after washing
Observation:Has sink 2 and 3 order reversed, must rinse soap off of items before sanitizing, wash, rinse, and sanitize in that order.
- Disclosure of menu items offered or served raw or undercooked
Observation:Does not have disclosure of items that may be undercooked, ie burgers (rarely ordered) and eggs.
- Discarding ready to eat, PHF/TCS food prepared on-site or opened commercial container held at 41°F for 7 days
Observation:Has salsa in individual portion cups that are past the 7 day use by date, discarded today.
