The sliced, cooked gyro meat in the walk-in cooler does not have the time provided. Label all cooling potentially hazardous food with the date and time of preparation/cooling began to be able to keep track of the total time cooling is taking place.
- Foods handled with minimal manual contact
Small, clear, single-service cups were being used to dispense diced onions and diced green peppers. Suitable dispensing utensils shall be used by employees or provided to consumers who serve themselves to avoid unnecessary manual contact with food. The cups have been removed.
- Food contact surfaces designed, constructed, maintained, installed, located.
The end of the rack in the prep cooler located to the left of the grill is worn on the end. Maintain all food-contact surface smooth, easily cleanable and in good repair.
- Plumbing installed and maintained
The backflow prevention device on the faucet of the mop sink is leaking when turned on. Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.
- Floors properly constructed, clean, drained
1) The floor in the walk-in cooler is worn. 2) A couple floor tiles are missing from the storage area upstairs. Maintain all floors smooth, easily cleanable and in good repair.
- Walls, ceilings, and attached equipment, properly constructed, clean
1) The interior surface of the walk-in cooler door is worn and the bottom is rusty. 2) A hole is present in the wall of the upstairs storage area. Maintain all walls, and attached equipment smooth, easily cleanable and in good repair.

The grate side (left side) of the grill is worn/rusty. The owner stated they will only put food inside of pans on top of the grate.
This establishment has been given final approval to operate under the guidelines of Chapter 10, Food Safety, of the Peoria County Code.
Check #1312 has been paid in the amount of $225.00 for the 2015 Food License.
This establishment will be billed for the 2016 Food License.
- Non-food contact surfaces designed, constructed, maintained, installed, located.
The edge of the counter/cabinet areas below the hand sink on the south side of the building are not sealed. All non-food contact surfaces shall be non-absorbent.

1) The floor in the walk-in cooler is worn. 2) The floor upstairs in the storage area has a few missing tiles. Maintain all floors in good repair.
- Walls, ceilings, and attached equipment, properly constructed, clean
1) The inside of the door of the walk-in cooler is worn and rusty. 2) The wall in the upstairs storage area has a few holes. Maintain all walls in good repair.
