Noted large amount of empty boxes; some broken down and some not broken down; throughout back area; some boxes left in mini storage aisle; and two boxes full of corn husks and hair. Per operator; the procedure is to brake down all boxes and place inside dumpster daily. Ensure to place boxes daily in dumpster to keep back area clear and to dispose of any corn trash immediately in dumpster in plastic bags to prevent attraction of pests.

Noted two light fixtures not operating in the prep areas and one fixture missing cover in back area. Fix light fixtures and provide missing light cover.

Noted hand sinks stocked and functional in prep area and in restroom.Refrigerators at below 40 F.Chlorine santizer in wiping cloths' bucket checked at 100 ppm concentration.Pots and pans with foods not being date marked; operator wil start doing date marking by next week; this will be required with new upcoming regulations.Ceviche used up within one to two days maximum.The Certified Food Protection Manager left employment about one month ago; Other employee completed an approved certtification class and obtained ServSafe certificate two months ago; must apply for WCHD's CFPM cetificate by 5/15/2015.

The items noted on the previous routine inspection conducted on 5/6/2014 have been corrected.The duro products made an packaged in a local permitted restaurant; met restaurant owner on site to confirm prodcut source; restaurant owner will provide letter of source verification to WCHD on 5/12/2014.Flan and cheese cake are purchased whole at local store. cakes are sliced and re-packaged on site; print date of repackage on labels. Keep labeling information from product boxes on site for list of ingrdients and manufacturer information.All products have been provided with lables.Owner contacted Waste Mangement and will have dumspter replaced within 2 weeks. Asphalt area being kept clean of liquid waste leakage in the mean time.

Observed flan in clam shell palstic containers in refrigerator with no lables and from nonverifiable source. Plastic bags of duro sheets and wheels with no lables; Must provide proof of approved source and have all prodcuts labeled with name of product and manufacturer information and list of ingedients. If proof can not be provided; products must be emoved and discontinued. Follow up on this item to be conducted on 5/8/2014.

Noted dumpster leaking liquid waste on asphalt ground at back of establishment creating a puddle that may cause bad odors and attract pests. No drain to sewer provided in this area. Clean ashpalt area and keep clean and have dumpster leak sealed or dumpster replaced.

Facility must have one Certtifed Food Protection Manager on staff full time who has completed an approved certification class by 3/25/2013.Ceiiling tiles with porous surfaces in food prep areas must be replaced with vinyl tiles and restroom door must be provided with self-closing device by 3/25/2013.Facility will share dumpster at back door with other tenants of mall. Ensure that common dumpster area is kept clean.Plans to open on 1/26/2013
