Observed sewage spill from ceiling. Permit to operate is suspended until facility is completely inspected by WCHD. Operator must contact WCHD at 328-2434 to request a reinspection. If facility is not completely ready to be operational at that time a $71 reinspection fee will be assessed for each additional reinspection thereafter. Under no circumstances shall any beverages be served from this location. Evidence exists that appears to show the bar has been operating recently. (beer kegs hooked up; soda lines charged; 3 comp sink containted water; etc.)
