- Observed deflector plate inside ice machine dirty with mold and mildew build up. All food contact surfaces must be clean and free of debris. Ice must be discarded and machine thoroughly cleaned and sanitized prior to serving ice again. *Corrected on site.- Ice bucket observed with a crack in the bottom that was "repaired" with clear packing tape inside the bucket and black duct tape outside the bucket. *Corrected on site. Bucket discarded.- Ice bags have the address of 7 Golden Gate facilities with check boxes but the specific store location for this property was not checked on bags that were filled for sale. Check the box with an indelible marker before placing out for sale.

Proper FIFO rotation practices in place.- Recommend increasing the frequency of ice machine maintenance and sanitation schedule. Check the interior of the machine frequently and utilize sanitizer or bleach solution to clean the the inside defelctor plate.- Check the appropriate store location on all bags of ice offered for sale before stocking the front freezer.
