Notes:-Handsinks and restroom clean and properly stocked.-Discussed hand washing policy and employee sick policy. Ensure employees sick with gastrointestinal symptoms ie. vomiting; diarrhea; jaundice; stay out of work at least 48 hrs after symptoms have stopped.-Walk in cooler at 40F. All product stored properly and dates good. Floor clean; and proper lights.-Reach in cooler at 38F. Discussed proper product labeling of sandwiches and burritos.-Ice machine was out of service at the time; but the inside was clean.-Observed some mouse droppings under the counter by the soda carbonator. Manager said she had just taken over the facility as of Febuary 20. When she arrived the facility had a large number of mice and numerous food items being stored on the floor. Also; the facility was very dirty. She called Clark Pest Control and they have been coming on a weekly basis to control the mice. I obtained a service description from Clark which is located in the file. The problem seems to be under control and the facility is clean with no food stored on the floor. I also checked a sticky trap that was empty. The operator is going to fax me a copy of their next PCO service.-Chemicals stored properly.-Outside garbage stored properly.

Observed inaccurate thermometer inside walk-in cooler. Thermometer registered 20 f inside cooler and 40 f at room temperature. Replace with accurate thermometer.

Observed one burnt out light in walk-in cooler and 2 unshielded lights in walk in. Replace burnt out light and either provide shields or "toughskin" style light bulbs to prevent glass from breaking.

Notes:Facility has started doing hot dogs. Facility must apply for WCHD Health Permit for a snackbar.Observed no problems with process. Ensure hot dogs are cooked to a mimimum of 140 f prior to offering for service. Hot dogs temped at 142 f - 201 f.Discussed wash; rinse; sanitize procedures. -facility has bleach/chlorine sanitizer and test strips -if facility wishes to use quat ammonia sanitizer; quat ammonia test strips will be neededRemove soap & paper towels off 3 comp sink and place back at handsink. Discussed importance of handwashing.*Discussed employee health. Food handlers illwith vomiting; diarrhea; fever should be exluded for at least 48 hours after symptoms stop.*Observed several out of date products in store. If operator wishes to sell out of date foods; operator must verify with manufacturer whether these dates are for safety or quality.

All d-con stations and spring traps were removed. Holes in back side of cabinets were sealed. No pest control has been established. No mice activity has been noted since the previous inspection per manager. No signs of mice activity noted at this time. Should any mice activity be encountered in the future; facility shall establish pest control service.

Snackbar Health Permit will be closed to history. If facility decides to engage in any hot-holding or cooking they will need to apply for another snackbar Health Permit.Grocery notes:All date marking codes checked ok; all food storage checked good.All refrigeration and freezers operating within acceptable limits.Ice bags properly labeled for traceback purposes.Restroom properly stocked with sanitary towels and soap.Three-door freezer checked at -15 degrees.
