    * Failure to operate a food and beverage establishment without a valid and current health permit for grocery store. This health permit has a permit renewal for June 2015. It is the responsibility of the owner/permit holder to pay permit fee on time and before the expiration date of the annual permit. Area health inspector has made several attempts to contact permit holder. It is the responsibility to update any changes in contact information. A notice was sent out to the last listed address on teh permit application. Failre to operate without a valid health permit will resulit in immediate cloure of the grocery store. The City of Reno business license will be notified if business continues to operate without a valid health permit to operate a grocery store. Business license may be revoked by city authority. Please remember to pay for renewal fee for permit annually.** Owner made payment over telephone and paid for permit and late fee. *** Corrected on site.

    All violations corrected at time of inspection. Thank you. Carpets have been removed from front two raised platforms and have been painted to be smooth and easily cleanable. The lighting in the walk in refrigerator still needs to be completed. Please continue to replace non-working lights.

    Handsink in restroom repaired and operating with hot and cold running water under mechanical pressure. Liquid soap and sanitary disposable towels stocked at handsink.Both restroom observed clean.All other violations still apply and remain uncorrected.

    200) Inaccurate thermometer noted at one door dairy cold holding case. Must provide accurate thermometer for cold holding case. Internal thermometer recorded at 60 F. Dairy milk temped at 37.8 F. Discussed with operator to provide an accurate thermometer at dairy case and monitor temperature to be at 40 F or lower.

    Observed handsink in only restroom broken. The drain pipe observed missing and handsink unable to be used for handwashing. * A mop sink is available to be used for handwashing in the interim. Operator will be repairing handsink by end of business day. ** This violation must be corrected within 48 hours. Failure to comply will result in immediate suspension of health permit.

    Observed toilet in restroom filthy. Restrooms observed dirty throughout. Grocery store must be kept in a sanitary condition to prevent vermin attraction and potential cross contamination. Operator shall clean and disinfect both restrooms from floor to ceiling.

This is a repeat violation from previous years 2013 and 2014. The carpeted floors shall be replaced to be smooth and easily cleanable. Observed floors throughout grocery store and under food shelving dirty. This is a repeat violation from previous years 2013 and 2014. Impervious hard nonporous floors shall be swept and wet mopped daily. Floors observed dirty and sticky underfoot.

Operator requested extension on 04/10/14 for abatement date of 04/25/14 and requested extension again on 05/02/14 for 05/30/14 Work completed on 05/30/14 and reinspection 2nd conducted on 06/06/14.*** All construction items completed *** Thank you.Carpeted ledge has been recarpeted and observed clean.The space betweent the walk in cooler and the ceiling has been enclosed and surface is smooth and easily cleanable.Store has been cleaned and rearranged for better flow.

Condensation water cannot drain to a garbage can. A building permit may be required to properly install floor drain for walk in unit. Contact Reno Environmental Control if drain may be extended to outside of premise at 775-334-2168 and/or building department at 334-2090.
